Invazívne meningokokové ochorenia – analýza súboru hospitalizovaných pacientov na detskom infekčnom oddelení DFN Košice v rokoch 2007–2014

Úvod: Invazívne meningokokové ochorenia (IMO) predstavujú akútne život ohrozujúce stavy. Ochorenie postihuje najmä deti do 4 rokov veku a dospievajúcich (15–19-ročných). Klinické formy meningokokového ochorenia sú veľmi variabilné. Najčastejšie prebiehajú pod klinickým obrazom sepsy, meningitídy, alebo ich kombinácie.

Materiál a výsledky: Autori analyzujú súbor 64 detských pacientov vo veku 0–18 rokov hospitalizovaných na Detskom infekčnom oddelení DFN Košice v rokoch 2007–2014 pre invazívne meningokokové ochorenie. Diagnóza ochorenia bola stanovená na základe mikrobiologických vyšetrení z likvoru a hemokultúry event. na základe klinického obrazu. Najčastejším klinickým obrazom bola kombinovaná forma sepsy s meningitídou (45 %), nasledovaná meningitídou (42 %) a sepsou (13 %). Najviac postihnutou skupinou boli deti vo veku od 0–4 rokov (83 %). V 15 prípadoch bola potvrdená Neisseria meningitidis séroskupina B a u 5 pacientov séroskupina C. Zaznamenali sme jedno úmrtie u dieťaťa s klinickým obra-zom purpury fulminans a komplikácie v zmysle vzniku sub-durálneho hygrómu u dvoch pacientov a ventrikulitídy u jedného pacienta.

Záver: Prognóza invazívneho meningokokového ochorenia je význame ovplyvnená včasným podaním antibiotickej liečby v nadväznosti na rýchlu a správnu diagnostiku ochorenia. Na ochorenie je potrebné myslieť nielen pri typických klinických príznakoch, ale aj v diferenciálnej diagnostike febrilných stavov, hnačkovitých, exantémových ochorení a pod. Napriek klesajúcej tendencii vo výskyte invazívnych meningokokových ochorení, ich priebeh ostáva závažný so smrtnosťou pohybujúcou sa okolo 10 %.

Invasive meningococcal diseases – analysis of hospitalized children at the department of infectious diseases DFN Košice in the years 2007–2014

Introduction: Invasive meningococcal disease is a serious, life-threatening infectious diseases. It is most common in children aged 0–4 years and teenagers (15–19 years). Clinical manifestations of the disease are very variable. The most common clinical courses are septicaemia, meningitis, or the combination of both.

Results: The authors analyze 64 children aged from 0 to 18 years, hospitalized at the Department of Infectious Diseases in the years 2007–2014, being diagnosed as suffering from a meningococcal invasive disease. The diagnosis was confirmed by a microbiological test of cerebrospinal fluid, hemoculture or according to the clinical course. The most common clinical course was the combination of sepsis and meningitis (45%), meningitis (42%) and sepsis (13%). The disease was most common with the children aged 0–4 years (83%). It was found out that 15 of isolates were serogroup B and 5 were serogroup C. One child with purpura fulminans died, two children had subdural hygroma complication and one of the children had suffered from ventriculitis.

Conclusion: The prognosis of invasive meningococcal disease is relatively good in case that the appropriate antimicrobial therapy is administered promptly up on presentation. It is necessary to consider the disease not only when the typical clinical symptoms occur, but also in connection with differential diagnosis of febrile stage, diarrhoea, exanthema, etc. Despite the fact that the incidence of the disease is declining, the course of the infection remains serious, with the case fatality ratio approximately 10 %.

Koincidencia echinokokózy a tuberkulózy (kazuistika)

Úvod: Tuberkulóza je chronické granulomatózne ochorenie s veľmi pestrými klinickými prejavmi, môže postihnúť hociktorý orgán, predovšetkým pľúca. Echinokokóza je zoonóza, kde človek je hlavným medzihostiteľom. Klinický obraz závisí na lokalizácii a veľkosti cýst. Tie sú najčastejšie lokalizované v pečeni a môžu byť rôzne dlhú dobu asymptomatické. V literatúre sú ojedinele popisované prípady súčasného výskytu obidvoch týchto ochorení.

Materiál a výsledky: Autori prezentujú kazuistiku 6,5-ročnej pacientky, ktorá bola prijatá do DFN Košice pre hypodenzné ložiská v pečeni. U dieťaťa boli prítomné vysoké zápalové parametre, pre možnú bakteriálnu etiológiu abscesov v pečeni ordinovaná parenterálna antibiotická liečba. Realizovaná laparotómia, z operačnej rany bol vykultivovaný Staphylococcus aureus. V rámci zobrazovacích metód bolo vykonané aj MRI vyšetrenie pečene – diferenciálne diagnosticky prichádzal do úvahy absces parazitárneho pôvodu. Sérologické vyšetrenia séra (antiechinokokové protilátky) boli pred a po operácii negatívne, histologické vyšetrenia vzorky pečene negatívne, ale dokázaná pozitivita PCR Echinococcus multilocularis. Tiež v histologickom vyšetrení zistená pozitivita PCR M. tuberculosis, dieťa bolo liečené 6 mesiacov antituberkulotikami.

Záver: Tuberkulóza aj echinokokóza sú ochorenia, pri ktorých klinické príznaky sú nešpecifické, diagnostika je sťažená. Správne stanovenie diagnózy a potvrdenie pôvodcu ochorenia je nevyhnutné pre liečbu a prognózu ochorenia.

Autori v diskusii poukazujú na diferenciálne diagnostické problémy u tejto pacientky a potrebu rozhodovania o liečbe echinokokózy, ak výsledky vyšetrení neboli jednoznačné.

Coincidence of echinococcosis and tuberculosis (Case report)

Introduction: Tuberculosis is a chronic granulomatous disease with a whole variety of clinical manifestations. It may affect whatever body organ, especially the lungs. Echinococcosis is a zoonotic disease. Humans are its main intermediate hosts. Clinical image depends on localization and size of cysts. The cysts are most frequently located in the liver and may be asymptomatic for various lenght of time. Cases of simultaneous occurrence of both diseases are rarely described in professional studies.

Material and results: The authors present a case of 6.5 years old patient admitted to the DFN Košice because of hypodense lesions in the liver. There were elevated parameters of inflammation by this child. Due to the reason of possible bacterial etiology of liver abscesses, parenteral antibiotic therapy was prescribed. Laparotomy was performed, Staphylococcus aureus was cultivated from operation wound. MRI examination of the liver was performed as a part of imaging methods – abscess of parasitic origin was considered from prospective of differential diagnostics. Serological tests of serum (antiechinococcosis antibodies) were negative before as well as after operation. Histological examination of liver samples were negative but positivity of PCR Echinococcus multilocularis was demonstrated. Positive PCR M. tuberculosis was detected also by histological examination and the child had been treated by antituberculitics for six months.

Conclusion: Tuberculosis and Echinococcosis are diseases with nonspecific clinical symptoms. Diagnosis is more difficult. Adequate assessment of diagnosis and confirmation of ethiologic agent are essential for treatment and prognosis of the disease.

The authors in discussion refer to differential diagnostic problems relating this patient and to the need for decision on the treatment of echinococcosis in case of ambiguous test results.

Odporúčania ESPID na liečbu gastroenteritíd – update 2014

V roku 2014 Európska spoločnosť pre detskú gastroenterológiu, hepatológiu a výživu (The European Society for Pediatric Gastroenterology, Hepatology, and Nutrition – ESPGHAN) spoločne s Európskou spoločnosťou pre detské infekčné choroby (The European Society of Pediatric Infectious Diseases – ESPID) vydali aktualizované odporúčania pre manažment akútnej gastroenteritídy (AGE). V obsahu sa venujú manažmentu gastroenteritídy v nemocničnom prostredí, enterálnej a parenterálnej rehydratácii, korekcii porúch elektrolytovej rovnováhy, komplikáciám a monitorovaniu priebehu ochorenia.

Akútna gastroenteritída je jedným z najčastejších ochorení u detí. V detskej populácii zostáva signifikantnou príčinou mortality i morbidity. Najčastejšou a najzávažnejšou príčinou AGE u detí vo všetkých európskych krajinách zostáva naďalej rotavírus. Najčastejším bakteriálnym patogénom, v závislosti od krajiny, je Campylobacter alebo Salmonella. Do prvej línie liečby gastroenteritídy patrí orálna rehydratácia hypoosmolárnymi roztokmi, ktorá má začať čo najskôr. Preferuje sa enterálna rehydratácia pred intravenóznou rehydratá-ciou. Lepšia účinnosť ultrarýchlej schémy intravenóznej rehydratácie oproti štandardnej schéme nebola potvrdená. Počas rehydratácie sa odporúča pokračovať v dojčení a pravidelnom kŕmení. Ku skráteniu trvania hnačiek môže prispieť užívanie špecifických probiotík ako Lactobacillus GG alebo Saccharomyces boulardii, diosmektidy alebo racekadotril. Antibiotiká sú efektívne v liečbe šigelóz a vo včasnom štádiu kampylobaktériovej enteritídy. Proti vracaniu je efektívny ondansetron, ale jeho rutinné používanie sa vzhľadom na možné kardiálne nežiaduce účinky neodporúča.

Tieto odporúčania sú dôležitým prostriedkom pre zlepšenie kvality zdravotnej starostlivosti a majú pomôcť lekárom na všetkých úrovniach zdravotnej starostlivosti, s prihliadnutím na to, že každý pacient je jedinečný.

ESPID guidelines for the therapy of gastroenteritis – update 2014

In 2014 The European Society for Pediatric Gastroenterology, Hepatology, and Nutrition (ESPGHAN) and The European Society of Pediatric Infectious Diseases (ESPID) jointly published updated giudelines for the management of acute gastroenteritis (AGE). The content of guidelinesis dedicated to the management of children with gastroenteritis in hospital, enteral and parenteral rehydration, correction of electrolyte imbalance, complications and monitoring the course of the disease.

Acute gastroenteritis is one of the most common diseases in children. It is still significant cause of mortality and morbidity in children. Rotavirus remains the most common and the most serious cause of AGE in children in all European countries. The most common bacterial patogen is Campylobacter or Salmonella, depending on country. The first-line treatment of gastroenteritis include oral rehydration with hypoosmolar solutions, which should start as soon as possible. Enteral rehydration is prefered to intravenous rehydration. Better efficiency of ultrarapid scheme of intravenous rehydration compared to the standard scheme has not been established. During rehydration it is recommended to continue breast-feeding and regular feeding. Administration of specific probiotics such as Lactobacillus GG or Saccharomyces boulardii, diosmectide or racecadotril may reduce the duration of diarrhea. Antibiotics are effective in therapy of shigellosis and in the early stage of Campylobacter enteritis. Ondansetron is effective agains tvomiting, but its routine use in view of the possible cardiac adverse effects is not recommended.
