Summary
Peripartum life-threatening bleeding is a significant cause of maternal morbidity and mortality worldwide. Rapid and massive blood loss requires quick and effective intervention. In addition to conservative treatments, we often have to use surgical interventions, including compression sutures. In our work, we describe the indication, technical conditions, and performance of a specific B-Lynch transversal compression suture. It is an effective surgical method, often in connection with an inverted or low-seated placenta during caesarean section. We compare the B-Lynch II with the B-Lynch I variant. If we consider the relative simplicity of the principle of the B-Lynch transversal suture effect and our own utilization of this modality, then we ask what is the cause of the persistent distrust and reservation when deciding to implement the suture in clinical practice and recommended procedures from the obstetrician. An explanation is offered that the introduction of this method into routine practice is limited mainly by a lack of knowledge, a lack of understanding the complexity of the intervention, and without a doubt, the fear of complications. Recommending the application of a compression suture only after the failure of other surgical methods can also lead to the discrediting of this method. However, we have experience that the application of a B-Lynch transversal suture as an ultimum refugium (before a possible indication of hysterectomy) is ineffective and often impossible in altered terrain and increasing blood loss. In a typical indication, we recommend using B-Lynch II as the method of first choice. We have our own set of 16 successful cases, which will be the subject of another communication.
