Diagnostické metody infekce Helicobacter pylori

Diagnostic methods for Helicobacter pylori infection

Helicobacter pylori infection is one of the most common chronic bacterial infections in humans and the principal etiological factor of chronic gastritis and peptic ulcer disease, as well as a major risk factor for gastric adenocarcinoma and gastric MALT lymphoma. The Maastricht VI/Florence consensus defines H. pylori gastritis as an infectious disease irrespective of symptoms. Reliable detection of the infection is therefore essential both for identifying infected individuals and for confirming successful eradication. Testing should be performed primarily in individuals in whom a positive result will lead to eradication therapy. This review summarizes the current options for diagnosing H. pylori infection, their diagnostic accuracy, advantages, and limitations. Non-invasive methods comprise the urea breath test using labelled urea, the stool antigen test, and serology. The first two detect active infection and are suitable both for primary diagnosis and for confirmation of eradication. Among stool antigen tests, laboratory-based assays achieve higher accuracy than rapid tests, and validated laboratory assays using monoclonal antibodies are preferred. Serology cannot distinguish active from past infection and is unsuitable for a test of cure, although it retains a role in settings where the sensitivity of other methods is reduced. Invasive diagnosis is based on biopsy specimens obtained at gastroscopy comprising of the rapid urease test, histology, culture, and molecular methods. Histology allows assessment of morphological changes of the gastric mucosa in addition to detection of the infection. Given the rising primary resistance of H. pylori to clarithromycin and levofloxacin in Europe, antimicrobial susceptibility testing, whether culture-based or molecular, is becoming increasingly important. The article focuses on the choice of method according to the clinical situation, on factors that reduce diagnostic accuracy, particularly the use of proton pump inhibitors, antibiotics, and bismuth-containing preparations, and on the rules for confirming successful eradication.

Souhrn

Infekce Helicobacter pylori patří mezi nejčastější chronické bakteriální infekce v lidské populaci a představuje hlavní etiologický faktor chronické gastritidy a peptického vředu i významný rizikový faktor adenokarcinomu žaludku a gastrického MALT lymfomu. Invazivní diagnostika vychází z bioptických vzorků odebraných při gastroskopii a zahrnuje rychlý ureázový test, histologické vyšetření, kultivaci a molekulárně biologické metody. Histologické vyšetření umožňuje vedle průkazu infekce posoudit také morfologické změny žaludeční sliznice. Rostoucí primární rezistence H. pylori ke klaritromycinu a levofloxacinu v Evropě zvyšuje potřebu stanovit antibiotickou citlivost kultivačními i molekulárními metodami.
