Obnovené neuroimunologické a likvorologické dny

Ve dnech 21.– 22. května 2010 uspořádala Neurologická klinika 3. LF UK a FN Královské Vinohrady ve spolupráci s Českou neurologickou společností ČLS JEP, pod záštitou jejího předsedy, doc. MU Dr. O. Kellera, Csc., děkana 3. LF UK v Praze, prof. MU Dr. M. Anděla, CSc., a ředitele FN Královské Vinohrady, MU Dr. M. Zemana, opět po čtyřech letech setkání likvorologů a klinických neuroimunologů. Sjezdu se zúčastnilo více než padesát odborníků z této oblasti i se zahraničními hosty z Rakouska a Slovenska. Tématem setkání byly novinky v likvorologii a nové terapeutické možnosti v léčbě roztroušené sklerózy mozkomíšní. Kongres proběhl v přátelském a vysoce pracovním duchu ke spokojenosti posluchačů i přednášejících.

Součástí tohoto kongresu byl i paralelně probíhající workshop, kterého se zúčastnilo padesát významných vědeckých a klinických pracovníků v oblasti neurorehabilitace z 18 zemí. Setkání zorganizovala Klinika rehabilitačního lékařství 3. LF UK a FN Královské Vinohrady ve spolupráci s Neurologickou klinikou a dalšími partnery (Rehabilitation in Multiple Sclerosis, Visegrad fund, CEROS, o.p.s., Metropolitní univerzita Praha, o.p.s. –  projekt tréninkových pracovišť, EuroMISE a Ústav informatiky Akademie věd České republiky, Česká lékařská společnost, Unie fyzioterapeutů ČR, Unie Roska, ČLS JEP –  Společnost pro rehabilitační a fyzikální medicínu).

Věříme, že akce přinesla všem zúčastněným potřebné informace, které ve svém důsledku pomohou zlepšit kvalitu péče o nemocné s roztroušenou sklerózou mozkomíšní.

Přehledová přednáška podává informaci o v současné době dostupných bio­chemických, cytologických, imunologických a mikrobio­logických metodikách vyšetření mozkomíšního moku –  likvoru. Jsou rozlišena základní a statimová vyšetření a parametry rozšířené specializované likvorologie. Je doporučen základní algoritmus laboratorního vyšetření likvoru s přihlédnutím ke konkrétní neurologické diagnóze.

Protilátky proti tau proteinu u roztroušené sklerózy

Úvod: Neurodegenerace u roztroušené sklerózy (RS) může souviset s autoimunitními pochody. Protilátky proti neurocytoskeletálnímu proteinu tau mohou tyto děje reflektovat. Proto cílem naší práce bylo stanovení protilátek proti tau proteinu v séru a mozkomíšním moku (MMM) u pacientů s RS.

Metodika: U 49 pacientů s RS a 47 neurologických kontrolních pacientů s různými jinými diagnózami (CD) byly v párových vzorcích séra a MMM stanoveny hladiny protilátek proti tau proteinu metodou ELISA. Intratekální syntéza byla vypočtena jako protilátkový index anti‑tau protilátek.

Výsledky: Intratekální syntéza protilátek proti tau proteinu byla významně vyšší ve skupině RS než u skupiny CD (p = 0,00001). Hladiny IgG protilátek proti tau proteinu jsou u obou skupin podobné jak v séru, tak i MMM. Specifické anti‑tau protilátky v séru i MMM korelují s celkovou koncentrací IgG v séru a MMM (r = 0,5). Nebyl nalezen žádný vztah mezi intratekální syntézou protilátek proti tau proteinu a pohlavím, trváním nemoci nebo mírou invalidity. Byla však pozorována souvislost intratekální syntézy s věkem pacientů s RS (r = – 0,4).

Závěr: Přítomnost protilátek třídy IgG proti tau proteinu v séru a mozkomíšním moku není pro roztroušenou sklerózu specifická, ale pacienti s RS vykazují jejich zvýšenou intratekální syntézu.

Evropská praxe v použití likvorového tripletu pro diagnostiku Alzheimerovy nemoci

Úvod: Biomarkery v mozkomíšním moku (MMM) mohou být užitečné v diagnostice demence. Přehled o jejich používání v klinické praxi po Evropě nebyl znám.

Metodika: Analyzovali jsme data z průzkumu o používání bio­markerů CSF v diagnostice demencí v celé Evropě pomocí dotazníku, který byl vyplněn zástupci 25 členských zemí Evropské federace neurologických společností (EFNS).

Výsledky: MMM beta‑amyloid, celkový tau a fosforylovaný tau protein jsou často vyšetřovány ve většině zemí (v 18 z 23 zemí). Nebyly zjištěny žádné závažné technické či etické problémy. Hraniční hodnoty se výrazně lišily v jednotlivých zemích a dokonce v rámci jednoho státu. V jednotlivých zemích byly zjištěny následující hraniční koncentrace: beta‑amyloid (medián 500 pg/ ml, rozpětí 300– 849 pg/ ml), celkový tau (367, 195– 450) a fosforylovaný tau protein (60, 40– 85).

Závěr: Vyšetřování MMM k diagnostice Alzheimerovy nemoci je používáno ve většině zemí Evropy. Vzhledem k různým normám v jednotlivých státech není jeho nadnárodní použitelnost snadná. Doporučujeme, aby si každá laboratoř zavedla vlastní normativní data. Měly by být zorganizovány multicentrické studie ke zjištění důvodů rozdílných výsledků.

Detekce zvýšené koncentrace lehké podjednotky neurofilament v mozkomíšním moku pacientů s roztroušenou sklerózou

Úvod: Rozpad neuronů a přetrhání axonů u roztroušené sklerózy (RS) vede k uvolnění cytoskeletálních bílkovin typických pro nervovou tkáň, např. lehké podjednotky neurofilamentového tripletu (NFL). Tento proces se může nepřímo projevovat v mozkomíšním moku (MMM) pacientů s RS.

Pacienti a metodika: U 17 pacientů s RS podle McDonaldových kritérií (průměr ± SD: věk 36 ± 8 let, 70 % žen) a 23 kon­trolních neurologických pacientů (35 ± 10 let, 65 % žen) jsme ELISA soupravami VIDITEST NFL firmy Vidia a současně jinou zahraniční soupravou změřili koncentrace NFL v MMM.

Výsledky: Koncentrace NFL v MMM byly významně vyšší (zhruba dvojnásobně) u pacientů s RS než u kontrolních jedinců srovnatelného věku a pohlaví (Vidia souprava: 700 ± 430 pg/ ml vs 360 ± 260 pg/ ml; p = 0,02). Podobné výsledky byly získány pomocí zahraniční soupravy. Koncentrace NFL všech 40 vzorků změřené oběma různými soupravami významně korelovaly (r = 0,4; p = 0,005).

Závěr: Cytoskeletální protein NFL specifický pro nervovou tkáň je významně zvýšený v MMM pacientů s RS, což může odrážet míru neuronálního postižení u RS.

Standard pro léčbu pacientů s autoimunitními nervosvalovými onemocněními intravenózním lidským imunoglobulinem a terapeutickou plazmaferézou

Standard vypracoval autorský tým Neuromuskulární sekce (NMS) Neurologické společnosti (NS) České lékařské společnosti Jana Evangelisty Purkyně (ČLS JEP) ve spolupráci s týmem pro vývoj Národní sady standardů zdravotních služeb a Národní sady ukazatelů zdravotních služeb. Předmětem standardu jsou dva imunomodulační postupy –  léčba intravenózním lidským imunoglobulinem (IVIG) a terapeutickou plazmaferézou (TPF), které mají důležité postavení v algoritmu léčby vybraných autoimunitních nervosvalových onemocnění: Guillainova‑Barréova syndromu (GBS), chronické zánětlivé demyelinizační polyneuropatie (CIDP), multifokální motorické neuropatie (MMN), myasthenia gravis (MG) a polymyozitidy/ dermatomyozitidy (PM/ DM). Důvodem vytvoření standardu je relativní náročnost diagnostiky, ekonomická a technická náročnost léčby i nezanedbatelné riziko komplikací. Autoři předpokládají, že zavedení standardu do praxe zlepší kvalitu péče vyloučením nestandardních a málo efektivních postupů. Ve shodě se současnými mezinárodními standardy je léčba IVIG a TPF doporučována jako léčba první volby u GBS (oba postupy jsou ekvivalentní), MMN (IVIG), CIDP (IVIG jako alternativa kortikosteroidů) a jako krátkodobá léčba zvl. u těžkých případů MG k navození remise, u exacerbace s rizikem myastenické krize a v přípravě k operaci (TPF nebo IVIG). Léčba IVIG je doporučována jako alternativní léčba druhé volby u DM (případně i PM) a dále u CIDP (pokud jsou kortikosteroidy neúčinné nebo kontraindikované); léčba TPF jako alternativní léčba u CIDP (v případě neúčinnosti kortikosteroidů a IVIG). Je doporučováno léčbu vybraných autoimunitních nervosvalových onemocnění konzultovat a v případě indikace léčby IVIG nebo TPF tyto pacienty léčit v Neuromuskulárních centrech (definovaných a stanovených výborem NS ČLS JEP). Pro účely sledování efektivity léčby standard definuje soubor kritérií a indikátorů kvality péče, které budou těmito centry sledovány a vyhodnocovány.

Dvouleté zkušenosti s léčbou natalizumabem

Úvod: Natalizumab se v ČR k léčbě pacientů s relaps‑ remitentní (RR) formou roztroušené sklerózy mozkomíšní (RS) používá od 1. 8. 2008. My jsme zhodnotili sledování pacientů našeho RS centra léčených tímto preparátem.

Metodika: 44 pacientů s RR formou RS, z toho 32 žen a 12 mužů. Hodnotili jsme délku onemocnění RS před zahájením terapie natalizumabem, rozložení pacientů dle poslední terapie před nasazením natalizumabu, vývoj EDSS v porovnání vstupní hodnoty a v průběhu terapie, relaps rate, vývoj MR nálezu a výskyt nežádoucích účinků u léčených pacientů.

Výsledky: U našich pacientů byla průměrná délka onemocnění RS před zahájením terapie natalizumabem 9,47 roků. Průměrná hodnota EDSS se během terapie snížila ze vstupních 3,89 na 3,70. V našem souboru jsme zaznamenali celkem 14 atak onemocnění. Celkem 63 % pacientů bylo bez nežádoucích účinků. Během léčby se vyskytla jedna alergická reakce, ostatní nežádoucí účinky byly nezávažné. Při kontrolním MR vyšetření mělo nové Gd+ léze pouze 9% pacientů, nové T2 léze 15 % pacientů a regresi nálezu jsme zaznamenali u 11 % pacientů.

Závěr: U našich pacientů jsme zaznamenali trend k poklesu EDSS. Při hodnocení klinické aktivity onemocnění bylo 75 % pacientů bez relapsu. Dle zhodnocení MR obrazu bylo 91 % pacientů bez nových Gd+ lézí a 85 % pacientů bez nových T2 lézí. Spektrum nežádoucích účinků odpovídalo dosud pozorovaným v rámci klinických studií. Celkem lze říci, že naše výsledky jsou srovnatelné s pozorováním pacientů během klinických studií.

Vyšetření mozkomíšního moku v diagnostice Alzheimerovy nemoci

Úvod: Histopatologické nálezy u Alzheimerovy nemoci (AN) v podobě neurofibrilárních smotků a senilních plak se mohou odrážet ve změněných koncentracích tří bílkovin v mozkomíšním moku (MMM). U pacientů s AN nacházíme v MMM zvýšenou koncentraci celkového a fosforylovaného tau proteinu a sníženou koncentraci beta‑amyloidu. Cílem práce je představit vlastní zkušenost AD Centra s vyšetřováním všech tří bílkovin v MMM (tzv. likvorového tripletu) v diagnostice AN.

Pacienti a metodika: U 26 pacientů s AN podle NINCDS‑ ADRDA kritérií (průměr ± SD: věk 76 ± 10 let, MMSE 18 ± 8 bodů) a 47 kontrolních neurologických pacientů (66 ± 8 let, MMSE 28– 30 bodů) jsme ELISA metodou změřili koncentrace likvorového tripletu.

Výsledky: Koncentrace tau a fosfotau proteinů jsou významně vyšší a naopak koncentrace beta‑amyloidu významně nižší u pacientů s AN než u kontrolních jedinců (tau protein: medián 549 pg/ ml, (minimum 199 pg/ ml –  maximum 1 720 pg/ ml) vs medián 209 pg/ ml, (54– 456); fosfotau protein 64 pg/ ml, (9– 172) vs 33 pg/ ml, (8– 79); beta‑amyloid 379 pg/ ml, (168– 1 300) vs 720 pg/ ml, (271– 1 243). Odlišení obou skupin pomocí likvorového tripletu má následující diagnostické charakteristiky (výsledky jsou uvedeny jako hraniční koncentrace se současnou maximální senzitivitou (Se) i specificitou (Sp): tau protein: 347 pg/ ml, Se 73 % a Sp 87 %; fosfotau protein: 42 pg/ ml, Se 77 % a Sp 77 %; beta‑amyloid: 510 pg/ ml, Se 65 % a Sp 81 %. Při individuálním rozboru byly u dvou pacientů s AN zjištěny normální hodnoty koncentrací všech stanovovaných ukazatelů.

Závěr: Abnormální nálezy v likvorovém tripletu mohou přispět k diagnostice AN. Největší diagnostickou přesnost k odlišení AN od normálního stárnutí má v našem souboru celkový tau protein. Jako každá pomocná vyšetřovací metoda má svá omezení, a proto je vhodné k upřesnění využívat také jiných informací z klinického vyšetření, neuropsychologického testování a zobrazování mozku.

Fingolimod – modulátor S1P receptorů, výsledky klinických studií

Fingolimod je první orální lék pokazující v klinických studiích efekt v léčbě roztroušené sklerózy mozkomíšní (MS). In vivo je fingolimod fosforylovaný na fingolimod‑ fosfatát, který je podobný receptorovému lipidovému modulátoru, sphingosin 1- fosfatátu (S1P). Fingolimod‑ fosfatát ovlivňuje nejméně čtyři z pěti S1P receptorů, označených jako subtypy 1– 5 (S1P1– 5). Tyto receptory hrají důležitou roli v imunitním systému, např. regulují vycestování lymfocytů z lymfatické tkáně do oběhu. Receptory S1P jsou rovněž exprimovány mnohými buňkami centrálního nervového systému (CNS). Fingolimod také přestupuje hemato‑likvorovou bariéru s možným přímým efektem na CNS. Profylaktické podávání fingolimodu v experimentu na zvířatech u experimentální autoimunitní encefalopatie (EAE) prokázalo úplnou prevenci rozvoje EAE. Terapeutický efekt, jež byl prokázán v experimentu na zvířatech byl dále testován v klinických studiích u nemocných s MS a přinesl velmi povzbudivé výsledky, které prezentujeme v této přednášce.

Poruchy štítné žlázy u pacientů s roztroušenou sklerózou: léčba tyreopatií a její vliv na základní onemocnění – předběžné výsledky prospektivní studie

Úvod: Hormony štítné žlázy jsou rozhodující jak pro myelinizaci, tak i pro demyelinizaci centrálního nervového systému. Roztroušená skleróza mozkomíšní (MS) a hypotyreóza mají řadu společných symptomů, např. únavu, svalovou slabost nebo deprese. Cílem této prospektivní studie je zjistit prevalenci tyreopatií u pacientů s MS, možný vliv DMD, imunosupresiv, kortikoidů a natalizubamu na jejich prevalenci a zejména vliv léčby tyreopatií na základní onemocnění.

Metodika: Prospektivní sledování po dobu dvou let, vyšetřeno bylo celkem 256 pacientů v období říjen 2009 až březen 2010 s RR, SP a PP formou MS. Bylo stanoveno vstupní EDSS skóre. Provedeno bylo vstupní laboratorní vyšetření na TSH, fT4, fT3 a protilátky anti‑TPO a anti‑Tg a sonografické vyšetření štítné žlázy. Pacientům s nalezenou poruchou štítné žlázy byla nasazena substituční terapie hypotyreózy, izohormonální terapie u chronické autoimunitní tyreoiditidy, supresní terapie nodozní štítné žlázy a nebo tyreostatická terapie tyreotoxikózy.

Výsledky: Zachyceno bylo celkem 49 pacientů s tyreopatií (19,1 %), z toho 38 (77,6 %) žen a 11 (22,5 %) mužů. Průměrný věk 40,5 let, medián 39 let, 2 pacienti s SP formou, 47 pacientů s RR formou MS. Průměr vstupního EDSS byl 2,5; medián byl 2, průměrná délka terapie DMD 5 let (bez DMD léčby 6 pacientů, z toho 1 gravidita, Tysabri 8 pacientů, Rebif 44 4 pacienti, Rebif 227 pacientů, Betaferon 9 pacientů, Copaxone 8 pacientů, Avonex 4 pacienti, IVIG 2 pacienti).

Zjištěné tyreopatie byly klasifikovány do pěti skupin:

  1. hypotyreóza 34 (13 %) z 256 pacientů z toho 23 (9 %) pacientů mělo přiměřený sonografický obraz,
  2. chronická autoimunitní tyreoiditida 18 (7%) pacientů, pozitivita anti‑TPO a anti‑Tg u 8 (3 %) pacientů, sonografický obraz tyreoiditidy u 17(6,5 %) pacientů, poruchy tyreoidálních funkcí u 12 (4,5 %) pacientů,
  3. poporodní tyreoiditida u 1 pacientky (0,5 %) s výskytem 8 měsíců po porodu,
  4. nodózní štítná žláza u 7 pacientů (2,5 %) a z toho FNAB u 2 (1 %) pacientů s benigní cytologií,
  5. tyreotoxikóza u 1 (0,5 %) pacienta.

Závěr: Naším cílem je sledovat možnou pozitivní změnu EDSS v závislosti na léčbě tyreopatií. Chtěli bychom otevřít otázku skríningu tyreopatií u všech pacientů s MS a rozvinout mezioborovou spolupráci.

Způsob a význam detekce oxidačního vzplanutí makrofágů v likvorové diagnostice postižení centrálního nervového systému

Zánět s oxidačním vzplanutím profesionálních fagocytů v centrálním nervovém systému představuje velice razantní proces, zpravidla závažné příčiny. Obecně známým příkladem je oxidační vzplanutí neutrofilních granulocytů při purulentním zánětlivém procesu zapříčiněném extracelulárními bakteriemi. Známe ale také neinfekční příčinu tohoto zánětlivého procesu v cen­trálním nervovém systému, kdy může dojít k jeho iniciaci po reperfuzi ischemického ložiska vzniklého následkem vazospasmů po atace subarachnoidálního krvácení. V každém případě se tento zánětlivý proces laboratorně projevuje granulocytární pleiocytózou s převahou neutrofilních granulocytů v cytologickém obraze likvoru a vysokým rozsahem anaerobního metabolizmu v likvorovém kompartmentu. Velmi vysoký rozsah anaerobního metabolizmu v likvorovém kompartmentu při absenci nebo při nevýznamné přítomnosti neutrofilních granulocytů v cytologickém obraze likvoru vede k oprávněnému podezření na oxidační vzplanutí jiných fagocytujících elementů v centrálním nervovém systému, a to makrofágů. Za přítomností zánětlivého procesu tohoto charakteru lze obvykle hledat závažnou infekční příčinu v podobě intracelulárních bakterií, kvasinek nebo plísní, či příčinu neinfekční v podobě nádorového procesu v oblasti související s likvorovým kompartmentem.

Autoři ve svém sdělení seznamují s podstatou zánětlivého procesu spočívajícího v oxidačním vzplanutí makrofágů v oblasti centrálního nervového systému související s likvorovým kompartmentem a předkládají ověřený algoritmus základního vyšetření likvoru umožňující jeho spolehlivou detekci.

V likvoru v současné době můžeme vyšetřovat řadu proteinů, od základních tříd imunoglobulinů, dále proteinů akutní fáze, složek komplementu, apolipoproteinů a strukturálních proteinů CNS jakožto markerů proběhlé tkáňové destrukce atd. Mezi relativně moderní imunologické parametry patří vyšetření cytokinů, které mají své nesporné výhody oproti výše popsaným proteinům, například ve srovnatelné koncentraci hladin v séru a v likvoru. Dále díky jejich poměrně podrobně popsaným charakteristikám lze dobře vystihnout základní fyziologické i patofyziologické děje odehrávající se jak v periferii, tak v centrálním nervovém systému, čehož využíváme ke komplexnímu hodnocení likvorového nálezu.

Transplantace krvetvorných buněk u autoimunitních onemocnění se zaměřením na roztroušenou sklerózu

Ně­kte­ré z autoimunitních onemocnění (AID) postihuje 5– 8 % populace. Mezi obtížně zvladatelná a těžce invalidizující onemocnění patří zejména systémová sklerodermie (SSc) a roztroušená skleróza (RS). Díky rozvoji transplantace krvetvorných buněk začala být zkoumána eskalace imunosuprese do myeloablativních dávek s podporou krvetvorných buněk (vysokodávkovaná imunoablace) a také ně­kte­ré nemyeloablativní způsoby intenzivní imunosuprese s cílem navodit remisi autoimunitného onemocnění prohloubením imunosuprese a vytvořením podmínek nového programování imunitní odpovědi (tzv. imunitní reset) s možností tolerance vlastních tkání. Recentně byla provedena analýza dobře dokumentovaných autologních transplantací u 900 pacientů. Z této analýzy vyplývá, že nejčastější indikací k vysokodávkované léčbě a transplantaci krvetvorných kmenových buněk jsou roztroušená skleróza a systémová sklerodermie. Z důležitých parametrů, které vzešly z této analýzy AID léčených autologní transplantací jako celku, jsou zejména celkové přežití (OS), přežití bez progrese (PFS) a mortalita spojená s transplantací (TRM). Celkové přežití je v této skupině 85 %, PFS 43 % a TRM 5 %. Ze 111 pacientů, kteří zemřeli, 43 pacientů zemřelo na progresi základního onemocnění a 59 na komplikace spojené s transplantací. Nejčastější příčinou úmrtí ve spojení transplantací byla infekce (46 %), kardiální toxicita byla příčinou úmrtí u 8 % ze 111 zemřelých pacientů. Jak vyplývá z počtu jednotlivých AID indikovaných k imunoablační léčbě, v případě RS je zatím nejvíce údajů o toxicitě a efektu terapie. Dosud publikované jsou studie fáze I– II s velikostí maximálně několika desítek pacientů. Studie z konce 90. let 20. století a studie publikované do roku 2007 zahrnovaly ve velké většině případů pacienty ve fázi sekundární progrese RS a byly orientovány především na popsání toxicity procedury (feasibility study), nicméně důležité informace o efektu léčby z těchto studií také vyplývají. Toxicita transplantace u RS byla nejprve spojena s TRM až 7 %, nyní se TRM pohybuje v rozmezí 1– 2 %. U většiny pacientů nebylo zaznamenáno dlouhodobé zlepšení v neurologickém deficitu, výsledkem však byla stabilizace. Společným kritériem pro všechny pacienty zařazené do těchto studií je selhání předchozí imunosupresivní a imunomodulační terapie. V těchto studiích je PFS ve dvou letech v rozmezí 61– 73 % a 36– 77 % ve třech letech. I do výše uvedených studií však byli, byť v menším počtu, zařazeni pacienti s relabující‑ remitující RS (RR‑ RS) a pacienti s nižším stupněm postižení (EDSS < 6,0). Výsledky těchto pacientů byly významně lepší a pokud bylo popsáno dlouhodobější zlepšení, bylo to většinou u tohoto typu pacientů. Proto je v posledních letech jednoznačný trend do nově koncipovaných klinických studií zařazovat pacienty s menším stupněm neurologického postižení, s výraznou zánětlivou aktivitou, kde lze očekávat větší a dlouhodobější efekt imunoablační terapie, a event. nástup regeneračních procesů v nervové tkáni. Recentně byla publikována pilotní studie amerických autorů, kteří zahrnuli do léčby 21 pacientů výhradně s RR‑ RS. Neurologické postižení se v době zařazení do studie pohybovalo v rozmezí EDSS 2,0– 5,0 (medián 3,1), tedy časné stadium neurologického postižení, u všech pacientů selhala předchozí imunosupresivní a imunomodulační terapie, jako přípravný režim byla zvolena nemyeloablativní dávka cyklofosfamidu s alemtuzumabem (i.v.), později byl alemtuzumab nahrazen ATG. Při mediánu sledování 3,7 měsíců došlo v 81 % ke zlepšení alespoň o 1 bod EDSS, pět pacientů (24 %) mělo relaps (ataku), ale další imunosupresí bylo dosaženo opět remise. Toxicita tohoto režimu byla zvladatelná, v souvislosti s podáním alemtuzumabu byla zaznamenána i imunitní trombocytopenická purpura (ITP). Randomizovaná studie proběhla v Evropě (ASTIMS), pacienti s EDSS mezi 3,5– 6,5 byli randomizováni mezi konvenční terapii (mitoxantron) a transplantací autologních PBSC po přípravném režimu BEAM + ATG. Pro nedostatečný nábor pacientů byla studie předčasně ukončena a výsledky jsou v této době analyzovány. V době této publikace lze říci, že imunoablační terapie s podporou autologních PBSC je u RS indikována v rámci klinické studie. Mimo klinickou studii je tento výkon indikován individuálně u velmi rizikových forem. Při indikaci musí být dodržena podmínka existence společné indikační komise složené z neurologa zkušeného v péči o pacienty s RS a transplantujícího hematologa zkušeného v problematice transplantace u autoimunitních onemocnění. U pacientů s velmi pokročilou RS nelze očekávat efekt této terapie. Orientací pro indikaci mohou být kritéria: 1. věk 18– 55 let; 2. vstupní EDSS ≥ 3,0 a ≤ 6,5; 3. sekundárně progredující forma s aktivními ložisky na MR CNS nebo bez nich, relabující remitující forma s aktivními ložisky na MR CNS; 4. jasná progrese v posledním roce léčby a selhání terapie 1. a 2. linie.

Oral cladribine for the treatment of multiple sclerosis – results from CLARITY study

Background: Cladribine provides immunomodulation through selective targeting of lymphocyte subtypes. We report the results of a 96‑week phase 3 trial of a short‑ course oral tablet therapy in patients with relapsing‑ remitting multiple sclerosis.

Methods: We randomly assigned 1,326 patients in an approximate 1 : 1 : 1 ratio to receive one of two cumulative doses of cladribine tablets (either 3.5 mg or 5.25 mg per kilogram of body weight) or matching placebo, given in two or four short courses for the first 48 weeks, then in two short courses starting at week 48 and week 52 (for a total of 8 to 20 days per year). The primary end point was the rate of relapse at 96 weeks.

Results: Among patients who received cladribine tablets (either 3.5 mg or 5.25 mg per kilogram), there was a significantly lower annualized rate of relapse than in the placebo group (0.14 and 0.15, respectively, vs. 0.33; p <0.001 for both comparisons), a higher relapse‑free rate (79.7 and 78.9%, respectively, vs. 60.9%; p <0.001 for both comparisons), a lower risk of 3- month sustained progression of disability (hazard ratio for the 3.5-mg group, 0.67; 95% confidence interval [CI], 0.48 to 0.93; ú = 0.02; and hazard ratio for the 5.25-mg group, 0.69; 95% CI, 0.49 to 0.96; p = 0.03), and significant reductions in the brain lesion count on magnetic resonance imaging (MRI) (p <0.001 for all comparisons). Adverse events that were more frequent in the cladribine groups included lymphocytopenia (21.6% in the 3.5-  mg group and 31.5% in the 5.25-mg group, vs. 1.8%) and herpes zoster (8 patients and 12 patients, respectively, vs. no patients).

Conclusions: Treatment with cladribine tablets significantly reduced relapse rates, the risk of disability progression, and MRI measures of disease activity at 96 weeks. The benefits need to be weighed against the risks.

Základné princípy kortikoterapie pri autoimunitných neuromuskulárnych ochoreniach

Úvod: Zavedenie kortikoterapie znamenalo veľký pokrok a zásadný prelom v prognóze pacientov so závažnými, v minulosti často smrteľnými, autoimunitnými neuromuskulárnymi ochoreniami (NMO). Napriek významnej efektívnosti je kortikoterapia prevažne empirická, neselektívna liečba, ktorá zasahuje do mnohých častí imunitného systému a pôsobí aj na iné orgány, čo sa môže prejavovať viacerými nežiaducimi účinkami. V klinickej praxi sa terapeuticky využívajú synteticky pripravené glukokortikoidy, ktoré majú imunosupresívny účinok na bunkovú aj humorálnu imunitu, ale aj ďalšie prospešné účinky (protizápalový, protialergický, antiproliferatívny, zníženie permeability a stabilizácia membrán). Hlavné indikácie pre kortikoterapiu sú myasténia gravis, polymyozitída, dermatomyozitída, chronická zápalová demyelinizačná polyneuropatia (CIDP), Lambert‑ Eatonov myastenický syndróm, získaná neuromyotónia, polymyalgia rheumatica, endokrinná oftalmopatia a vaskulitídy s neuromuskulárnymi príznakmi.

Základné princípy kortikoterapie pri autoimunitných NMO:

  • Dostatočne vysoká dávka prednizonu (1– 1,5 mg/ kg telesnej hmotnosti) v úvode liečby.
  • Liečba prednizonom musí byť dlhodobá. Preto je potrebné prejsť na režim alternujúcich jednorazových dávok. Táto schéma podávania prednizonu zabezpečuje optimálny terapeutický efekt a významne redukuje riziko vzniku vedľajších prejavov.
  • Riziko vzniku nežiaducich komplikácií je potrebné znižovať profylaktickými opatreniami (strava s nízkym obsahom sodíka, glycidov a vyšším prísunom bielkovín; blokátory H2 receptorov alebo inhibítory protónovej pumpy; preparáty vápnika a vitamín D; podľa potreby substitúcia draslíka; vhodné je sledovanie kostnej denzitometrie).
  • Dlhodobo liečených pacientov je nutné dispenzarizovať, kontrolovať objektívny stav, monitorovať výsledky laboratórnych vyšetrení a výskyt nežiaducich prejavov.

Ku klinickému zlepšovaniu autoimunitných neurologických ochorení dochádza obvykle v prvom mesiaci kortikoterapie, ale u niektorých pacientoch môže dôjsť k zlepšeniu až po niekoľkých mesiacoch liečby. Doba podávania prednizonu potrebná k zaisteniu trvalého liečebného efektu je individuálne rôzne dlhá. Základnou podmienkou pre postupné znižovanie prednizonu je stálosť optimálneho klinické zlepšenia ochorenia.

Najčastejšie príčiny zlyhania kortikoterapie sú v nesprávne realizovanej liečbe:

  • nedostatočná úvodná dávka prednizonu,
  • nedostatočné trvanie kortikoterapie,
  • predčasný začiatok znižovania prednizonu,
  • rýchla redukcia dávok prednizonu.

Nesprávne vedená liečba často vedie k závažným exacerbáciam ochorení, ktoré môžu pacientov vitálne ohroziť, napr. vznikom myastenickej krízy. U viacerých pacientov s autoimunitnými ochoreniami je potrebná k trvalému zabezpečeniu optimálneho liečebného efektu aj niekoľkoročná kortikoterapia. V týchto prípadoch je indikovaná kombinovaná imunosupresia prednizonom a azatioprínom, prípadne iným imunosupresívnym preparátom. Prednizon umožňuje rýchly nástup terapeutického efektu a azatioprín je výhodný u tých pacientov, ktorí vyžadujú dlhodobú alebo trvalú imunosupresívnu liečbu. Azatioprín má pri dlhodobom podávaní oveľa nižší výskyt vedľajších prejavov ako prednizon. Udržovacia imunosupresívna dávka azatioprinu 1– 2 mg/ kg telesnej hmotnosti je pacientmi dobre tolerovaná aj dlhé roky.

Vedľajšie príznaky kortikoterapie: Relatívne často sa vyskytujú cushingoidná facies a nadváha, ktoré však pri znižovaní a najmä po vysadení liečby ustúpia. Z ostatných nežiaducich prejavov sa vyskytujú akcelerácia katarakty, osteoporóza, steroidný diabetes, steroidná myopatia, vzácne steroidná psychóza a krvácanie do tráviaceho traktu. V profylaxii nežiaducich účinkov je významná diéta s nízkym prísunom glycidov, sodíka a vysokým prísunom bielkovín, preparáta vápníka, blokátory H2 receptorov, prípadne inhibítorov protónovej pumpy, dispenzarizácia a monitorovanie stavu pacientov.

Záver: Kortikoterapia je veľmi účinná v liečbe viacerých autoimunitných NMO. Má však mnoho úskalí a rizík, preto jej vedenie a dispenzarizácia pacientov musí byť v odbornej kompetencii erudovaných špecializovaných neurológov (myológovia, neuroimunológovia).

Nové aspekty v léčbě interferonem beta1b

Roztroušená skleróza mozkomíšní je nejčastější chronické zánětlivé onemocnění u mladých jedinců. Významná klinická progrese se objevuje u poloviny pacientů již během 2.– 3. roku onemocnění a během deseti roků polovina pacientů přechází se sekundárně chronicko‑progresivní fáze onemocnění. Tyto závažné skutečnosti jsou navíc doprovázeny rozvojem kognitivní dysfunkce asi u 65 % pacientů a ztrátou zaměstnání u 45 % pacientů v první dekádě onemocnění. Časné patologické změny u roztroušené sklerózy mozkomíšní jsou důsledkem fokální zánětlivé demyelinizace s axonálním postižením. Axonální transakce je ireverzibilní a rozsáhlá již v časné fázi onemocnění. Pokud axonální ztráta překročí kompenzační rezervy centrálního nervového systému, je doprovázena ireverzibilní klinickou progresí. Interferon beta‑1b (Betaferon) v in vitro a in vivo studiích vykazuje přímé i nepřímé protektivní účinky na nervové buňky CNS, redukuje počet hypointenzních T1 black holes na MR v korelaci s mozkovou atrofií a zlepšuje axonální integritu měřenou MR spektroskopií. Interferon beta‑1b ovlivňuje zánětlivý proces u roztroušené sklerózy a působí i neuroprotektivně. Optimální zahájení léčby je v období prvních příznaků u tzv. klinicky izolovaného syndromu (CIS). Studie BENEFIT významně přispěla k uvedenému, neboť se jedná o prospektivní randomizovanou studii prokazující dlouhodobý efekt časné léčby na konverzi do klinicky definitivní roztroušené sklerózy (CDMS). Interferon beta‑1b (Betaferon) redukuje riziko přechodu do CDMS o 37 %, oddaluje prokázanou progresi o 1,5 roku a zlepšuje kognitivní funkce měřené PASAT testem. Přispívá k zachování vysoké kvality života a je dobře tolerován s příznivým bezpečnostním profilem.

Physical rehabilitation in multiple sclerosis uses a variety of techniques and methods and it is difficult to clearly define its content. Neurophysiologic methods are used since 60ties last century (for example Bobath concept, proprioceptive neuromuscular facilitation, Vojta reflex locomotion) till present. Originally these methods worked on the hierarchic model of the locomotion control and they were applied in physiotherapy as a so called facilitation approach. The attitude to the locomotion control kept changing with the development of neuroscience and imaging methods and it was demonstrated by different models. One of the latest ones is a so called system model which forms the base for the so called task‑oriented therapeutic approach, or in a wider concept problem solving approach, focusing on “specific” problems of an individual person/ client/ patient. Application of mention methods changed in this sense and new methods have been developed (for example Motor relearning programme). Both these most frequently used approaches are based on senso‑motor learning. Therefore they have in common the fact that they apply different stimuli to achieve better locomotion and function. The facilitation approach puts the accent on manual application of stimuli (by propriocetive and exteroceptive stimulation, in Bobath concept e. g. by so called handling, in Vojta reflex locomotion by stimulating of so called initiation zones in precisely‑defined positions) with the aim to facilitate and improve a given locomotive function, movement pattern or to start a locomotion programme, while the quality of execution is carefully controlled. The task‑oriented approach makes use of mainly behavioral impulses or stimuli and a patient learns by repeating a given specific function in different environment/ under different conditions; the ability to carry out a specific function is more important than the quality of the execution.

Application of the International Classification of Functioning, Disability and Health (ICF) to rehabilitation of persons with multiple sclerosis

The International Classification of Health and Disability (ICF) is a globally agreed upon framework to understand and classify the typical spectrum of problems in functioning of persons given their environmental context. Multiple sclerosis (MS) is a disease that can follow a variety of clinical courses and is unpredictable in terms of prognosis. Impairments due to MS can produce disabilities and functional limitation that impact on everyday activities of persons with MS (PwMS) and cause restrictions in daily life participation. ICF can thus be a useful approach to understanding the short and long term limitations of the MS disease. Classification with ICF can be used to indicate those barriers and facilitators to participation that globally affect PwMS and need to be targeted at social and political levels, as well as, variables that are more unique to the individual PwMS and should be targeted at the person’s physical and environmental level. In rehabilitation the use of the ICF approach may facilitate a patient‑oriented, comprehensive and multidisciplinary management of the short term and long term rehabilitation needs of PwMS. Rehabilitation based on the ICF approach can impact on all levels of needs, from the impairment level (Body Structures) to overall quality of life (Activity and Participation), and along the continuum of care ranging from the acute hospital to rehabilitation facilities and the community. The application of the ICF checklist and the MS specific core sets to the PwMS, and resultant findings in the existing literature, will be discussed. Unpublished pilot data from the application of the ICF checklist to PwMS will be presented.

Various philosophical ways of approaching illness

The body is not only a spatial formation but it is also a formation in time. It is constructive to investigate the phenomenological concept of the connection between the body and the world through intentionality, i.e. through the phenomenology of internal temporal perception. This provides a new approach to physical illnesses. It is necessary to examine the internal perception of time in a non‑Cartesian way. Urimpression (primal impression) metamorphoses into retention, which is retained in the flow of our cogitations. Due to the interest in urimpression, protention is formed from retention which is the lens through which the implicitly of our living world, our own Dasein, can be seen. In other words, if we want to understand the patient, we must join in their protentional plan, their noesis. Noesis carries inside it that which we find as noema. It is a phenomenological approach to illness.

From neuroscience to neurorehabilitation – new concepts in physical therapy

It has long been known that the success of neurological rehabilitation relies on simultaneous engagement of both internal and external resources: patient motivation and effort on one hand, and stimulation and exercise on the other. Evidence from wide areas of neuroscience research may now be used to maximize treatment potential of both inner and outer influences. More than twenty years ago, basic neuroscience research has elucidated the conditions which lead to enhanced effectiveness of synaptic connections among neurons forming functional networks in adult mammalian brain. The observed stimulation conditions provide inspiration to techniques facilitating impaired movement and improving patients‘ neurological deficits. Furthermore, movement facilitation methods have traditionally focused on somatosensory afferent stimulation. More recently, other stimuli and modalities, such as therapist‑patient interaction, music, rhythm and dance have become more deeply and extensively explored. Whether they facilitate movement directly or mediated by the emotional limbic system of the brain, these techniques provide another avenue for motor rehabilitation research. Finally, recently developed functional brain imaging methods allow noninvasive visualization of sensorimotor networks and their engagement by different physiotherapeutic techniques in individual patients. Imaging findings then need to be corroborated by clinical and neuropsychological tests. Employment of these conceptual and methodological developments may improve the search for new and optimized treatments in neurorehabilitation.

Goals of rehabilitation in MS

Goals and goal setting are essential components in the rehabilitation process. A goal can be defined as “how things will be at some specified time in the future”, or as “a desired state that requires both action and effort”. However, there is no standard use of terminology in relation to goal setting in rehabilitation for either persons with MS or other disabilities. A number of advantages can be achieved by goal setting. Setting goals e. g. influences positively on behaviour change and improves adherence to rehabilitation programs. Moreover, goal setting encourages individuals with MS to define their own goals and thus improve their autonomy and engagement to rehabilitation interventions. The acronym SMART has been accepted to specify the characteristics of an ideal (appropriate) goal. The acronym letters refer that goals need to be specific, measurable, achievable, realistic, and timed. Because writing goals for persons with MS may be time consuming and difficult, it provides a practical means to define goals in a concrete manner and is useful in order to overcome such problems. Well‑defined goals are particularly needed for goal attainment scaling (GAS), which is a method for rating goal achievement. The method is easy, quick, patient specific, and applicable to most situations. An Australian study (2008) showed that GAS suits well for persons with MS with wide range of disability, and reflects person‑centered goals beyond simple gains in independence for ADLs (as measured by the FIM and Barthel Index). Other studies dealing with goal setting in MS have shown – somewhat alarmingly – that persons with the disease and members of rehabilitation team do not necessarily agree well on the specific goals for an in‑patient rehabilitation stay or that goal setting meetings may be guided by professional dominance. Clearly, additional research is crucial to strengthen the evidence base of goal setting in rehabilitation for persons with MS.

Common motor symptoms in multiple sclerosis (MS) are paresis and fatiguability, leading to walking difficulties. As patients consider gait as a valuable function, it is important to include standardised and valid measurements in study designs and clinical practice. Different walking capacity tests and patient‑reported outcomes are currently used, impeding comparison between studies. Common short walking capacity tests are the T25FW and 10m test which can be executed at comfort­able or maximal speed, and with standing or flying start all influencing recorded gait speed values. The differential value of longer walking capacity tests (2 and 6 minute walking tests), is not known. Recent studies demonstrated reliability of 6MWT and high correlation with fatigue, as well as high correlations with habitual walking performance in persons with mild disability. Besides, general fatigue unrelated to physical or cognitive performance is a common complaint. Often, patients report to be more fatigue with time of day possibly influencing performance on gait tests. Small studies didn’t confirm this, but applied short walking capacity tests only. The need for consensus on gait outcome measures has been addressed on a CMSC meeting in 2007 (Consortium of MS Centers, USA). Propositions for fatigue and gait measures were made, but not grounded on comparative scientific data. Members from the RIMS network (Rehabilitation in MS: European network of MS Centers, www.rims.be), specifically the Special Interest Group on Mobility, have collaborated: 1 multi‑center study investigated the differential value of short and long walking capacity tests, and the MSWS‑12 while another study investigated the effects of time of day on perceived fatigue and physical walking performance, via repeated measurements during one day. The Modified Fatigue Impact Scale was used to document level of fatigue while the Rochester Inventory was used to hourly measure energy level. 10/ 8 centers and 189/ 114 patients participated to the studies. Results will provide guidelines for gait outcome measures and time of assessment according to different levels of ambulatory disability, and be published in 2011.

Critical discussion on the use of outcome measures in individual patients

Aim: To critically discuss the use of outcome measures in individual patients.

Aim of measurement: Concerns the constructs of interest and the purpose of the measurement. The International Classification of Functioning Disability and Health (ICF) model can provide the relevant constructs. Two purposes can be distinguished: 1. to discriminate between normal and aberrant (discrimination); and 2. to measure changes over time (evaluation).

Clinimetric properties: For the purpose discrimination a cut‑off, and the sensitivity and specificity for this cut‑off should be known. For the purpose of evaluation an outcome measure should be able to measure the change that is relevant for the patient (Minimally Important Change [MIC]) reproducibly. Reproducibility data can be used to calculate the Smallest Real Change (SRC) for individual patients, which is the change that is needed to overcome the measurement error. Only when the SRC is smaller than the MIC, an outcome measure can be used to measure changes in individual patients.

Practical aspects: Issues like, how much (treatment) time can be used for measurements, complexity of the scoring system, what is needed to make the scores readily available for the clinicians, what are the consequences for the organization and what costs are allowed, may determine whether or not systematic outcome measurement can be implemented.

Discussion: The construct of interest and the purpose of measurement determine which kind of outcome measure to select. For the purpose discrimination outcome measures with defined cut‑off scores with known sensitivity and specificity are suitable. Sufficient responsiveness for application in individual patients is the most difficult clinimetric property to fulfill. Practical aspects determine successful implementation.

Measuring physical function of mobility aids users – a modern psychometric theory approach

Physical function is an important aspect of health related quality of life. In the USA the National Institutes of Health (NIH) funded Patient Reported Outcomes Measurement Information Systems (PROMIS) recently used modern psychometric theory, including Item Response Theory (IRT) to develop a number of item banks, including an item bank for measuring physical function (PF). NIH funded additional work to make the PROMIS physical function bank appropriate for users of mobility devices (such as wheelchairs and grabbers). New and modified items were administered to 758 persons with MS (n = 274), SCI (n = 268) or arthritis (n = 216) who use mobility devices. New and modified items were calibrated to the PROMIS metric using a graded response model (an IRT model). Test information for revised and original PROMIS‑PF banks was compared to levels of functioning in persons who use AT. The additional items substantially improved the precision of the bank in measuring participants with physical function challenges. Because the new items do not specifically reference mobility devices, the increased precision provided by the items improves measurement of all individuals at the lower end of physical function. The improved item bank has excellent psychometric properties and is available free of charge to researchers and clinicians. Computer adaptive tests (CAT) will be available in late 2010 through the PROMIS Assessment Center located at www.nihpromis.org. Targeted short forms (SF) for upper body and lower body are available and can be administered as paper or pencil instruments or CAT. Using these new tools people with and without mobility impairments can be measured using the same item bank (using SFs appropriate for target population) with scores directly comparable to that of individuals without physical limitations.

Reflex Locomotion according to Vojta (VRL) is diagnostic and therapeutic concept based on activation of global locomotion patterns, which are saved in CNS. If the function of locomotion system of patient was reduced or disturbed because of illness or injury, in CNS innate basic locomotion patterns are provoked, theirs activation enriches spontaneous by illness or injury reduced motoric. The global patterns are activated in defined initial position, which give already afferent input to CNS and provoked by specific stimulations applied on stimulations points of precise location on the body and defined pressure directions. The reflex creeping –  prone position, reflex turning –  supine, lateral position and the first position –  kneeling position are used for therapeutic purpose. Different duration (20– 60 min), frequency (once a day to once a week) and choice of therapeutic positions are always individual and are chosen according patients kinesiological assessment, diagnosis, subjective problems and patients reaction on therapy. The therapy influences postural and locomotors function as well as sensory and neurovegetative functions. The efect of VRL in patients with multiple sclerosis is described by Laufens et al. According to our experience the therapy has positive effect on the posture quality, the patient’s mobility, decreases spasticity, ataxia, dyskinesia, leads to improvement of balance, sensibility, circulation, swallowing and articulation, sphincter function, digestion and other autonomic functions. Patients perceive the positive effect of therapy very well. The long‑term regular therapy leads to patient’s self‑sufficiency and prolonged work activity.

Proprioceptive neuromuscular facilitation

Dr. Hermann Kabat and Maggie Knott developed in the mid‑1940’s a concept of treatment whose name is Proprioceptive Neuromuscular Facilitation (PNF). Its underlying philosophy is that all human beings, including those with disabilities, have untapped existing potential (Kabat, 1950). Dr. Kabat’s conceptual framework for PNF came from his experience as a neurophysiologist and physician and Maggie Knott as a physical therapist could bring her clinical experience. They were complementary in the development of a new concept. Their goal was to develop a hands‑on treatment approach that enabled clinicians to analyze and assess a patient‘s movement while at the same time facilitating more efficient strategies of functional movement. So it is important to recognize that PNF is not just a treatment approach, rather, it is a tool that allows for simultaneous assessment and treatment of neuromuscular dysfunction. From its beginnings, PNF has successfully integrated many of the concepts of contemporary neurorehab interventions. The Philosophy and Basic Principles of PNF, together with the specific spiral and diagonal patterns, make up the cornerstone of PNF. PNF also includes motor learning and functional retention of newly learned activities with the repetition of a specific demand, the use of the developmental progression of motor behavior that enables patients to create and re‑create strategies of efficient functional movement and the bio­­mechanical and behavioral analysis of motor control. All activities within PNF intervention are directed towards a functional goal and are relative to the environment in which the goal is to be achieved. PNF can be applied in the rehabilitation as well as in the acute care, in different clinical applications (neurology, orthopaedic, geriatric, paediatric, sport rehabilitation…). The PNF‑concept is spread over the world (e. g. with post‑graduate courses) and constantly developed through the international PNF‑association (www.ipnfa.org).

Perfetti approach

Neurorehabilitation was characterized during the first half of the 20th century by therapeutic actions of type reflex. In the 70s, the Italian neurologist Carlo Perfetti, unsatisfied with the results obtained in physical therapy, especially in the upper limb of stroke patients, began to interpret movement as a chain of processes that begins in the memory and attention of the individual. The basic hypothesis of Perfetti’s Neurocognitive theory presumes that cognitive function activation can determine the development of the own aptitude for the individual relate to the environment, both in normal conditions (learning) and in pathological conditions (rehabilitation). This Neurocognitive theory considers reeducation as a learning process in pathological conditions, that is to say, a process that is closely dependent on the activity of cognitive functions. From that point of view, our reeducational work as physical therapists will be characterized as being an intervention directed to guide the patient towards the activation of cognitive functions, in order to improve or facilitate the maximum recovery of the damage caused by the injury. Cognitive processes are considered fundamental in order that patients can re‑acquire a correct motor organization. Quality and effectiveness of the recovery depend on the type of cognitive activation processes and on its modality. The appearance of a muscle contraction after a paralysis has a poor significance if the patient can’t understand the world through his body in movement.

The stimuli application in physiotherapy in way that activate “programs of the CNS”

There are nervous system genetically determined factors of motor behavior –  muscle function encoded in motor patterns –  automatic reactions of the motor system to afferent stimulation that mature during the course of postural ontogenesis. Their evidence could be explain based on existence of central pattern generation (CPG) of locomotion on spinal and brain stem level. Above the brain stem level, there are neuronal mechanisms that select which CPG is to be turned on at any given moment. In therapy we use the stimuli: adaptive resistance (intensity of resistance is adapted to muscle reaction to provide fluent movement) in combination with quick maximal strengthening of muscle group before the start of movement (reciprocal inhibition) and word command in the before movement start are applied. The stimuli are applied to use interneuronal network (principles of convergention and divergention) the most effectively –  if necessary to sum up, facilitate, inhibit stimuli, use them in sense of feed back or feed forward –  and bring the information (afferent input) into the brain with aim to activate interplay of brain structures and to activate motor program that evoke global reaction of the body (co‑contraction of agonist and antagonist and functional centration –  the best possible distribution of the load at the artricular surfaces of the whole body, stabilisation in the sagittal plane). The stimuli are applied in different functions, e. g. standing up, sitting down or stepping. These functions are repetitively practicing within a practice session with aim to modify and strengthen cortical engram –  a widely distributed group of neurons comprising multiple smaller functional grouping that tend to fire synchronously during a movement, and to start plastic and adaptive processes.

The principles of the taskoriented approach in the rehabilitation of neurological diseases

Rehabilitation needs to be regarded as a problem‑solving process with its own specific focus on activity limitation and its own sets of goals, namely optimization of a person’s social participation and well‑being. Rehabilitation should aim to maximize a patient’s behavioural repertoire; in other words by giving them the skills and equipment needed to minimize the limitation on those activities they need or wish to undertake. The rehabilitation processes and interventions may be influenced by the theoretical basis (whether based on evidence or not) underlying the clinical approach to problems. Of course much rehabilitation practice is atheoretical, sometimes described as ‘pragmatic’ or ’empirical’. There is, however, a range of theories (e. g. reflex theory, hierarchical theory, motor programming theory, systems theory) underpinning the approach of the different disciplines, and a range of models for assessment and intervention. The aim of this presentation is to provide health professionals with basic knowledge to decide which model/ s is more useful as a structure for the assessment and treatment for a given patient.

Is the Bobath Concept still a relevant therapy to be used with patients suffering from a lesion of the central nervous system?

The Bobath approach is widely discussed nowadays since some people still perceive it as passive and mostly oriented on inhibition of abnormal tone and movement using the hierarchical model of motor control. More new therapeutical approaches have developed and recently also modern technologies like virtual reality and robotic devices are used. Where is the Bobath Concept standing in this development? What are the principles of the Bobath concept and are they still valid and useful in treatment of patients with brain damage? These are the questions to be discussed in the presentation. The Bobath Concept started to develop in the middle of the 20th century. At the beginning it was based on the hierarchical model of motor control according to the neurophysiological knowledge of that time. There were also passive inhibitory techniques and positions to decrease high tone since spasticity was at that time perceived as one of the major problems of these patients. However, Bobath explained their therapeutical approach in an interview many years ago as “…a whole new way of thinking, observing, and interpreting what the patient is doing and then adjusting what we do in the way of techniques to see and feel what is necessary and possible for them to achieve. We do not teach movement. We make movement possible.” (Bobath, 1981) It also has to be clear that the Bobath Concept has never been a rigid technique or method but continuously changing one. Generally it can be described as a way of observing, analyzing, and interpreting task performance. To do this it is necessary to assess the client’s potential which means considering the tasks or activities which could be performed by the person with a little help and therefore possible for that person to achieve independently where possible. Within the Bobath Concept various techniques and handling are used and Bobath always advocated to use what works best with each individual patient.

Dual tasking

During the last 15 years there has been an increasingly interest in “dual tasking” and the numbers of publications about dual tasking are numerous. Not surprisingly, the majority of these publications concludes that there is an interference of tasks when we perform them simultaneously. In general there is a decrement in performance under dual tasks conditions compared with the single tasks performances. In spite of this a number of articles suggest that we physiotherapist’s should include dual tasking in our assessments to get an impression of how the patients react under these “real live” conditions. The last couple of years the call for dual tasking as treatment approach has increased as an attempt towards a more “ICF’s participation‑level” attitude. In my presentation I will explain the theory about Limited Conscious Capacity (LCC) as the main cause of this negative interference. I will present some ideas about the choice of tasks and how to change this negative interference into a positive one by implementation of stimulating, motivational and arousal factors.

Exercise therapy for achieving functional goals in multiple sclerosis

Exercise therapy is one of the concepts used for Persons with Multiple Sclerosis (PwMS) to maintain and to rebuild their lost or diminished functional capabilities. Within the ICF model of Physical Rehabilitation in Multiple Sclerosis we are interested to see if any exercise intervention at the different levels of the ICF model would give the PwMS benefits for the superior levels. If we for example try to build up muscle strength (Impairment level) does this reflect in an increased walking capacity (Activity level) and will this stimulate PwMS to increase the physical activity level and go out for shopping or sport activities (Participation level)? Another question relates the existence of guidelines for PwMS for participating in exercise therapy or sports programs? In this abstract we discuss existing evidence regarding exercise therapy and factors of success for setting up exercise and/ or sport programs and maintaining adherence. In a meta‑analysis, Motl & Snook (2006) looked for the effects of exercise therapy on walking capacity in Multiple Sclerosis. The number of studies included was 20 with in total 600 PwMS and a frequency of 66 outcome measures included. From those 66 outcome measures 54 showed improvements in both objective measures and patients reports. The effects of exercise therapy on walking are positive but small (effect size of 0.2). A further analysis of these results shows no improvements on walking capacity in samples with only “primary progressive” MS. Regarding the setting of the interventions no improvements were shown in unsupervised home‑based therapy neither in combined home and supervised exercise facilities. No additional improvements were found after the first three months of training. Another meta‑analysis (Motl et al, 2008) looked after the effects of exercise therapy on Quality of Life, including fatigue. The number of studies included was 13 with in total 484 PwMS. Different exercise inter ventions were reported (aerobic and non‑aerobic) and 77 of the 109 outcome measures used were positive (effect size of 0.23). They found significant effects but still also large proportion of unexplained variance. Regarding the effect of exercise therapy on general fatigue mixed results are found at first sight, which seem dependent on the scales that were used. For endurance training for example there was always a positive influence with the Modified Fatigue Impact Scale and almost never a positive effect with the Fatigue Severity Scale (Dalgas et al, 2008). We can conclude that evidence exists for supporting exercise therapy for PwMS. On the other hand we do know that the physical fitness of PwMS is lower and that no negative side effects (relapses) will be caused with well supervised exercise‑ and sport programs.

The German approach of MS rehabilitation – results of a survey

A survey among German neurological rehabilitation units (n = 183) was performed to get an overview about treatment structures and concepts. Ninety questionnaires could be analysed of which 20 clinics reported to be specialised in MS with units of about 30 beds. Nearly all units cover a broad expertise of therapeutic approaches. Physiotherapy and exercise training are the dominating therapeutic approaches. Two third of the patients undergo neuropsychological examination, cognitive training is performed in all units. Group education programmes are administered for about 1 hour per week in all centres. Less than 20% of all units perform written goal‑setting and very few use disease specific standardized outcome measures. Lack of therapeutic personnel and of physicians and an increasing economic pressure as well as a lack of research resources for rehabilitation seem to be risk factors for the fact that rehabilitation in Germany might not be further developed. Therefore further scientific evidence and implementation studies are needed to secure the long‑term availability of rehab in MS.

Rehabilitation in Poland

The situation in Polish rehabilitation is changing very dynamically nowadays. These changes occur not only in an educational matter but also in a law matter. Very intensive works on a low about the occupation of a physiotherapist are still in progress. These regulations would interchangeably approve physiotherapy as an independent branch of other medical occupations and gather all regulations included in many decrees into one regulation. At the moment, to become a physiotherapist after secondary‑school leaving examination one should graduate from a three‑year long studies to get B.A. degree and then two‑year studies for M.A. degree. What is more, after three years of work in physiotherapy, it is possible to get a specialization by taking part in a unique training program. 150 colleges and universities have an accreditation for educating physiotherapists, which admit 13,000–15,000 first year students a year. Even though not all students graduate, nevertheless, the situation on the labour market is really hard. Physiotherapists in Poland can are employed in hospitals, outpatient clinics, sanatorium, rest houses or they can open they private consulting‑rooms.

Statistical analysis of questionnaire on physical rehabilitation in multiple sclerosis

We expect data from approximately 30 participants from approximately 15 countries. Questionnaire will be analyzed by various statistical methods and results will be presented on the workshop.

How to conduct a methodologically sound clinical rehabilitation intervention trial – experiences from an ongoing randomized study

Inpatient rehabilitation in MS typically consists of short‑term interventions. Hence, it is important to evaluate the effectiveness of these. Ideally the evaluation should be based on a randomized controlled trial (RCT), which presumably is the most important method to advance knowledge of rehabilitation. In 2009 a clinical study entitled “The effects of electrical stimulation and strength training in MS assessed by electromyography and walking measures” was commenced at the Masku Neurological Rehabilitation Centre. The study in question is a prospective single blinded RCT. The objective is to compare the effects of two different physiotherapeutic methods (electrical stimulation and strength training) on the EMG‑activity of the ankle muscles and on walking mobility. Data collection started in October 2009 and is currently ongoing. The aim is to collect data on 40 persons with definite MS of an EDSS score between 3.0 and 6.0 (inclusive). The two interventions are strictly standardized, and are part of inpatient rehabilitation programs lasting at least for 17 days. Several criteria and guidelines have been developed to assess the reliability and validity of RCTs. The steps of the above study will be reviewed in the light of these. Both methodological as well as practical aspects will be discussed to demonstrate how it is possible –  with limited financial and other resources –  to strive to overcome risks for biased results.

Functional Electrical Stimulation – an orthoses or a rehabilitation opportunity?

A new Functional Electrical Stimulation (FES) device has recently become available in the UK that is more discreet and easier to apply than the more traditional foot drop devices available. This device is triggered using a tilt sensor allowing the user to wear a range of footwear of their choice as a foot switch is not required and has additional potential for gait re‑education. In the UK the National Institute for Health and Clinical excellence (NICE) supported the use of FES for drop foot of central neurological origin and yet funding for such devices remains at best patchy throughout the UK (NICE Interventional procedure guidance 278 January 2009). At the Bristol & Avon MS Centre (BrAMS) we have been carrying out an audit of the use of FES in MS to inform future attempts at procurement. Between October 2009 and March 2010, 24 people with MS have been referred for assessment and inclusion in the audit. Fourteen people were suitable for FES and were able to take a WalkAide® device home for a three month trial, four were not suitable and six are waiting to be seen. Activity levels were recorded before and during FES use using an ActivPal® activity monitor and participants were questioned about lifestyle, quality of life, falls, pain, walking effort/ distance and patients’ perception of gait quality. Timed walk (10 m) was recorded in clinic and gait changes recorded by observation. The results and outcomes of this audit to date will be presented but also exploration of the role of FES in rehabilitation as the device allows an increased tolerance of exercise, a decrease of compensatory movements and improved dynamic posture realignment. Does this provide an opportunity for therapeutic input and are there other outcomes that are valuable to the patient and practitioner?

Optimal intervention for improving walking in multiple sclerosis – treadmill or strength training?

Aim: The effect of exercise is shown both in strength and treadmill training. The aim of this study is to evaluate the effectiveness of these two different training interventions on gait parameters.

Methods: Ongoing randomized clinical study. Thirty persons with MS will be recruited, 19 participants are included. The main criteria for inclusion is affected gait (EDSS ≤5.5). Outcome measures are walking parameters, static balance, postural sway while walking, isometric strength and rate of force development on m. quadriceps and work economics/ aerobic capacity. The participants fill the SF‑36 score before and after intervention.

Intervention: The training period will last for eight weeks, three workouts per week. Treadmill group (TG) has three separate parts, seven minutes each focusing on uphill, higher velocity and walking pattern. Strength training group (STG) has four exercises for legs and one for back and arm muscles. The focus is on strengthening legs in calf exercises, side leg lifts in standing position and leg press in lying position. All exercises for legs are done unilaterally.

The intervention is planned to be finished in autumn 2010 and the results during spring 2011.

Balance disorders in the transition from focal movement to quiet standing – a pilot study in multiple sclerosis patients

Introduction: Multiple sclerosis (MS) is the most common progressive neurological disease in young adults. Because of the variable distribution of demyelination throughout the central nervous system, people with MS may experience coordination, strength, sensation and balance disorders. Multivariate assessment is necessary to explore the balance system and to develop a fall prevention program. The first aim of this study is to describe balance impairment in a sample of subject with MS in tasks related to activities of daily living.

Methods: In this descriptive study we assessed a consecutive sample of 24 MS subjects aged 51 ±10 years (mean ± sd). We also assessed 24 healthy subjects aged 57 ±15 years. We proposed three different tasks: standing up from a chair (S), taking a step forward (F), and bending forward‑left (B). To this purpose a force platform was used to study the transition between focal movement (e. g. bending) to regime balance (quiet standing). We used the postural stabilization test to provide three quantitative parameters (Fig. 1): T, related to the time necessary to get to regime posture; Y, related to the “instability rate” at the beginning of the stabilization phase, and Yinf, related to residual regime instability.

Results: The results of the study is summarized in Fig. 1.

Discussions: The results suggest that subjects with MS had worse performances in stabilizing erect posture with respect to healthy subjects. Yinf, significantly higher for MS and comparable for all tasks (p <0.05), confirmed that long term balance was poor for patients, but not task‑dependent. For tasks S and F: Y0 values were lower for MS suggesting that subjects with MS were less unstable at focal movement end, maybe due to a greater cautiousness in accomplishing the task for fear of a balance loss. In spite of this, parameter T indicated that the time needed to stabilize was longer highlighting their difficulty in postural stabilization after focal movement.

Balance training in individuals with multiple sclerosis in Iceland

Multiple sclerosis (MS) is a degenerative neurological disease and the most common neurological disease in young people (20– 35 years of age). Today about 350 individuals in Iceland are living with the disease. Balance problems are one of the first complaints in about 50% of individuals that have been diagnosed with MS, and these symptoms have high impact on the individuals’ health, independence and social participation. The balance problems with impaired postural control results in decreased balance confidence, decreased performance and skills in daily activities, and can lead to vicious cycle of immobility, including decreased daily activities and social participation. Other serious health conditions are falls resulting in bone fractures, but frequency of falls is high in individuals with MS, 45– 63%. Increased frequency of bone fractures and decreased bone health is also a comorbid problem compared to healthy people at the same age and sex. The effect of balance impairment can therefore have great impact on quality of life of these individuals. Recent clinical studies on balance training in groups of individuals with MS have shown improvements in balance and postural control with less falls. Also, community based balance exercise programs have shown to be effective and practical in other neurological conditions. This knowledge leads us physical therapists at Reykjalundur rehabilitation center in Iceland to implement a balance‑circuit group‑training program for individuals with MS in association with the Icelandic MS patient association. The results of this experiment are increased balance and improved functional gait in participants. Also, the participants have shown to have good compliance and are very pleased with the program. A formal study on this balance‑circuit group‑training program for patients with MS in Iceland is needed.

Method MFK Science in physiotherapy

The MFK Method combines manual work of physiotherapists with computer system (hereafter only “the expert MFK System”). The expert MFK System processes the medicinal data which the therapist uses while assessing the diagnosis and therapy of functional disorders of the motor system. The expert MFK System provides the physiotherapist with comprehensive information about various kinds of interrelation of the patient’s body motion functional disorders. After the initial interview as well as during each next visit of a client, the therapist carries out an examination evaluating myokinetic activation. He or she always compares right and left side. The specific change of myokinetic activation compared to the standard activation is recorded in the expert MFK System in the accurately determined location. The mykonetic activation examination results are assessed by an algorithm based on the interdependencies of various body systems (nervous, muscular, articular and spinal). After a thorough assessment of the acquired objective and subjective findings form anamnesis and the examination the physiotherapist selects suitable physiotherapeutic technique from the scale of techniques provided by the expert MFK System (stimulation of the system of reflexive points, massage of muscles, stimulation of trigger points – TrP and tender points –  TP, mobilization, electrotherapy, exercise etc.). The procedure and location of the selected technique interventions is elaborated according to the actual finding and carried out by the expert MFK System. An accurate application of the selected technique is entirely in the hands of the physiotherapist. After the each accomplished therapy, the physiotherapist carries out a checkup of myokinetic activation of those muscles which were marked as changed against the standard during the examination.
