News Prim. Václava Adámková: People mistakenly believe that there are enough antibiotics and that they will always handle infections
According to the World Health Organization (WHO), by 2050, infections untreatable with antibiotics could become the leading cause of death, surpassing cardiovascular and oncological diseases. Rising antimicrobial resistance was one of the main topics at the VIII Congress of Clinical Microbiology, Infectious Diseases, and Epidemiology (KMINE 2022), held in September in Prague. The topic was also the focus of the expert block ‘ATB stewardship,’ where epidemiologist Petr Smejkal, infectologist Aleš Chrdle, and microbiologist Václava Adámková discussed possible solutions to this global problem. We asked Adámková to summarize insights into the presented issues for us.
News Nucleic Acid Amplification Tests in the Diagnosis of Sexually Transmitted Diseases
Based on 81 scientific papers published between 2010 and 2021, experts from Johns Hopkins University in Baltimore have prepared an overview of the current epidemiology, diagnosis, and treatment of sexually transmitted diseases (STD). We summarize information on the diagnosis of gonorrhea, chlamydial infections, M. genitalium, trichomoniasis, genital infections by HSV-1/2, and HPV using nucleic acid amplification tests (NAAT), the most well-known method being PCR. NAAT shows high sensitivity and specificity in testing for these diseases.
News Can we properly manage osteoporotic fractures?
Osteoporotic fractures are associated with increased morbidity and mortality and often lead to an overall deterioration in the quality of life. We should not forget to diagnose osteoporosis in patients after fractures and subsequently set appropriate therapy.

News Teriflunomide vs. Dimethyl Fumarate: Does Their Classification into Different Treatment Lines Have Merit?
Teriflunomide is one of the first-choice medications in the treatment of multiple sclerosis (MS) patients, not only in the Czech Republic. Conversely, the use of dimethyl fumarate is more limited here compared to other countries. Did the data from real-world clinical practice published this year actually demonstrate differences in the efficacy of both medications, and thus justify their classification?
News Is Six Months of Adjuvant Targeted Therapy Sufficient for Early Breast Cancer?
The inclusion of adjuvant targeted therapy with chemotherapy for early HER2-positive breast cancer significantly improved patient outcomes. Since 2006, based on data from registration studies, the gold standard for adjuvant therapy has been 12 months of trastuzumab administration. However, similar effects were observed in a smaller study with just 9 weeks of treatment. The presented clinical trial therefore investigated the non-inferiority of shorter treatment duration (6 months).
News Why does the heart enlarge? Myosin mutation and hypertrophic cardiomyopathy may be the culprits
The genetic basis of hypertrophic cardiomyopathy was discovered only about a quarter-century ago. Since then, researchers have gathered a wealth of knowledge about the effects of causal mutations. Today, new drugs are available that directly target the molecular basis of the disease. The following text explains how mutations in heart muscle proteins are linked to myocardial hypertrophy.
News Long-term Efficacy and Safety of Mavacamten in the Treatment of Obstructive HCM
Interim results of the MAVA-LTE study provide the latest data on the long-term safety and efficacy of symptomatic treatment with mavacamten for obstructive hypertrophic cardiomyopathy (oHCM), with the longest follow-up period nearly 2.5 years. For clinical practice, the long-term reduction of left ventricular outflow tract (LVOT) obstruction and good tolerability of the treatment are particularly favorable.
News Comparison of Efficacy of Bisoprolol and Metoprolol ZOK in Patients with Mild to Moderate Hypertension – Results of the CREATIVE Study
Suppression of increased activity in the sympathoadrenal system is currently one of the main goals of antihypertensive therapy. Beta-blockers play an important role in regulating the sympathetic nervous system. The CREATIVE study evaluated the efficacy of bisoprolol and metoprolol ZOK on blood pressure and heart rate in patients with mild to moderate primary arterial hypertension.
News Long-term Safety of Dupilumab in Adult Patients with Moderate to Severe Atopic Dermatitis
Moderate to severe atopic dermatitis (AD) often requires systemic therapy. The long-term safety of dupilumab in patients with this condition was evaluated in an open-label extension (OLE) of the LIBERTY AD study, the results of which were recently published.
News Subcutaneous Immunoglobulin Substitution in Ig-Naive Patients with PID – Available Efficacy and Safety Data
The prevalence of primary immunodeficiencies (PID) is estimated at 1:10,000. Below, we summarize the available findings on the efficacy and safety of subcutaneous immunoglobulin substitution in Ig-naive patients (i.e., without prior immunoglobulin substitution).
News Incontinence Dermatitis - Common, Yet Often Overlooked Diagnosis
Incontinence dermatitis is an inflammatory skin reaction with a prevalence between 3 and 25%, caused by prolonged skin contact with urine or feces. It is classified among contact irritant dermatitis. It causes significant discomfort to patients and can also require difficult and prolonged treatment.
News Comparison of Clinical Outcomes of TKA Implantation: ATTUNE vs. SIGMA
The ATTUNE total knee arthroplasty (TKA) system was introduced to the market in 2013 as a successor to the popular SIGMA system. The new type of knee endoprosthesis is designed to provide better stability and mobility than the previous implant. Several studies have evaluated the clinical outcomes of the implantation of both types of endoprostheses.
News Morning Myopic Shift and Glare in Patients with Advanced Fuchs Endothelial Corneal Dystrophy
A newly published study by a German team analyzes the morning and afternoon differences in various parameters in patients with Fuchs endothelial corneal dystrophy to determine which of them contribute to subjective visual difficulties in the morning.
News Effectiveness of Bisoprolol in Patients with Cardiac Arrhythmias – A Systematic Review of Studies
Last year, a Franco-Romanian team published a study that conducted a systematic review of existing evidence on bisoprolol in the context of arrhythmia treatment. The authors noted that bisoprolol, like other beta-blockers (BB), is used for a variety of cardiovascular diseases, but it is not mentioned in the European Society of Cardiology (ESC) guidelines for managing patients with ventricular arrhythmias and preventing sudden cardiac death.
News Liposomal Ozonated Oil in the Treatment of Eye Infections
Liposomal ozonated oil is a new ocular antiseptic with antibacterial, anti-inflammatory, and regenerative properties, making it suitable for the prevention and treatment of eye infections and as an adjuvant in wound healing. In June 2022, Spanish authors published a review summarizing findings on the mechanism of action and results of preclinical and clinical studies with ozonated oil in the treatment of eye infections and in the regeneration of the ocular surface. It also highlights the benefits of ozonated oil compared to conventional antiseptics such as povidone-iodine and chlorhexidine.
