Bilateral acquired stenosis of the choans and velopharynx following radiotherapy for nasopharyngeal carcinoma

Choanal and velopharyngeal stenoses are rare late complications of radiotherapy for nasopharyngeal carcinoma. We present the case of a 65-year - -old patient who developed progressive bilateral nasal obstruction, anosmia, and rhinorrhea without epistaxis, occurring four years after the completion of radiotherapy. The diagnosis was established based on nasal endoscopy, histopathological analysis, and computed tomography. The patient underwent endoscopic endonasal surgical recanalization of the choanae and velopharynx, followed by stent calibration using a soft tubular silicone catheter. The procedure led to marked symptomatic improvement. No evidence of restenosis was observed at 24-month follow-up.
