In this article, the historical circumstances of the formation of advance directives are analysed. We discuss the meaning of the text and the importance of its context for its interpretation. There is a great strain between the static text of an advance directive and the dynamics of the context, through which the patient’s text goes through. There are potential risks but these can be reduced by contextual interpretation.
