Summary: The paper analyzed 60 cases of granulomatous myringitis. In 77% of them there were diseases of the eardrum membrane without association with previous surgical therapy and in 23% after previous surgical therapy. The authors analyze pathogenesis of this less known nosological entity and differential diagnosis with emphasis to the determination of early and precise diagnosis. Conservative therapy is a method of choice, but surgical treatment can be used in complicated cases. The success rate of independent conservative treatment in our group was 76%. Ten patients underwent a surgical treatment – myrinoplasty. A re-operation was necessary in one patient. However, in spite of the re-operation the problem of stenosis of auditory canal and blunting of eardrum membrane still persists. In cases of granulomatous myringitis as a complication of healing after tympanoplastic operation, healing has always been reached with conservative therapy. Even nowadays, despite a wide palette of local otological preparations, granulomatous myringitis represents a frequently underestimated therapeutic problem.
