Transforming the life path of „floating seniors“

Overview

The management of discharges of the elderly from hospital currently involves the provision of follow-up care, particularly for multimorbid and at-risk elderly patients. The study aimed to: (a) compare the course of hospital stays of elderly patients over 80 years of age readmitted to the Department of Geriatrics of the University Hospital Olomouc in 2010 and 2020; (b) ascertain whether or not the “life journey” of the elderly changed; and (c) identify the types of facilities into which patients were discharged in 2010 and 2020. The data were obtained from patients’ medical records. In 2010, out of a total of 461 discharged patients, 138 elderly patients were discharged into their homes. In 2020, it was 68 out of 408 patients. In 2010, the life journey of 10 discharged elderly patients was monitored; it was found that all of them were readmitted to various hospital departments within six months of discharge (a total of 23 readmissions). Similarly, among 10 selected elderly patients discharged in 2020, 22 readmissions were noted. However, the patients’ life journey changed. The proportion of patients discharged into retirement homes increased from 12% in 2010 to 50% in 2020. Thus, retirement homes tend to become their real homes.
