Large multilocular thymic cyst in a patient with multiple autoimmune diseases: a case report

Overview

Mediastinal cystic masses represent a wide spectrum of nosological entities. Multilocular thymic cyst is a very rare form. A 59-year-old man with a history of pulmonary sarcoidosis with mediastinal lymph node and skin involvement, ulcerative colitis, and rheumatoid arthritis, was diagnosed to have a sharply demarcated tumor resistance in the thymus region by CT scan. It was surgically removed along with a subpleural lymph node. Grossly, it was a large multicystic lesion measuring 14 × 9 × 3 cm. It microscopically consisted of multiple cysts lined by flat, cuboidal, and squamous epithelium. There was intense mixed inflammatory infiltration in the surrounding stroma. A diagnosis of multilocular thymic cyst was done. The lymph node revealed non-caseating granulomatous lymphadenitis. Despite its rarity, multilocular thymic cyst should be included in differential diagnosis of cystic tumorous masses arising in the anterior mediastinum. Through this case, we show that this is especially true for the patients with autoimmune disorders, as it almost always coexists with them.
