Overview
Bleeding from the anterior part of nasal cavity, from injured anterior ethmoidal artery, may be potentially life-threatening. If conservative measures fail to stop the bleeding, there are several possibilities of its surgical management: ligation (coagulation) of the anterior ethmoidal artery by using external approach via medial orbitotomy, or transnasal endoscopic ligation (coagulation) of the artery, which has been a subject of controversy in the past few years. The authors present a case of severe posttraumatic anterior epistaxis with external approach anterior ethmoidal artery coagulation as a method of choice, and discuss current treatment trends. Despite of the rapid development of endoscopic surgery technique, the conventional external approach method of ligation (coagulation) of injured anterior ethmoidal artery remains the method of first choice. Transnasal endoscopic techniques are still a matter of discussion, with need of further clinical evaluation.
