Overview

The paper describes anatomical point of view, operation procedure, endoscopic anatomy and practical application of minimal trans-emissary retrosigmoidal approach with the application of endoscopy. The first experience is documented by case reports of four patients in the following indications: section of vestibular nerves, neurovascular conflict and small vestibular schwannoma. The operation indication goals were fulfilled in all cases and in the case of vestibular schwannoma we succeeded in saving normal functions of n. intermedius and n. facialis as well as normal hearing. This operation approach with the application of endoscopy extends our ability in surgery of cranial base.

Key words:
minimal trans-emissary retrosigmoidal approach, small vestibular schwannoma, vestibular neurectomy, neurovascular conflict.
