Druhá generace antipsychotik (= atypická antipsychotika) má řadu výhod hlavně z hlediska nízké afinity k extrapyramidovém systému. Jejich podávání však může vést k vedlejším účinkům, které mohou být problematické hlavně z pohledu interního lékaře. Je to zejména přírůstek hmotnosti a s tímto částečně související narušení lipidového a glukózového metabolizmu. V neposlední řadě některá atypická antipsychotika vedou k prodloužení QT intervalu se všemi dalšími možnými důsledky.
