Atypical localisation of pyoderma gangraenosum in patient with ulcerative colitis

Overview

Autoři prezentují případ 61letého muže s ulcerózní kolitidou a s extraintestinálními projevy onemocnění ve formě pyoderma gangraenosum. Vícečetné kožní defekty, které se objevily v netypické lokalizaci (rozsáhlé postižení obličejové a vlasaté části hlavy) u pacienta s chronicky aktivní formou ulcerozní kolitidy se komplikovaly bakteriální kontaminací meticilin rezistentním Staphylococcus aureus. Po zavedené parenterální výživě, kortikoterapii a cílené antibiotické terapii se výrazně zlepšil subjektivní i objektivní stav pacienta, snížila se frekvence stolic, vymizela příměs krve ve stolici, ustoupily teploty a laboratorně došlo ke snížení aktivity nespecifického zánětu střevního. Endoskopické vyšetření střeva však potvrdilo nález chronicky aktivní ulcerózní kolitidy s ulceracemi a přemosťujícími polypy. Pacient byl indikován k totální kolektomii, kterou však odmítl.
