Liver involvement in celiac disease

Celiac disease is an autoimmune disorder occurring in individuals with a genetic predisposition who consume gluten. The manifestation of celiac disease is diverse, presenting either as a classic gastrointestinal phenotype or througt extraintestinal manifestations. Celiac disease can be associated with liver diseases of various etiologies, including autoimmune liver diseases, celiac hepatitis, metabolic dysfunction-associated steatotic liver disease, or metabolic dysfunction-associated steatohepatitis. Hepatic involvement is usually mild and reversible, although in rare cases, it can progress to liver fibrosis, cirrhosis, and even liver failure. This article presents the case of an 18-year-old patient with hepatopathy of unclear etiology, later diagnosed with metabolic dysfunction-associated steatotic liver disease in the context of ongoing celiac disease. After the patient was placed on a strict gluten-free diet, the hepatic enzymes had normailzed and the hepatophathy had improved. The aim of our article was to highlight a relatively frequent manifestation of celiac disease, emphasize the nessesity for acurate diagnosis and screening in patients with unexplained liver damage, and to point out the fact that in certain cases, adherence to a gluten-free diet may contribute to the improvement of of liver damage.
