Prenatal diagnosis of ovarian cysts, management and pregnancy outcomes

Overview

Objective: To analyse prenatal diagnosis, management, and pregnancy outcomes in fetuses with an ovarian cyst and compare our results with foreign literature.

Methods: Retrospective analysis including fetal ovarian cysts diagnosed in the years 2017-2021 in our centre. The size and appearance of the cyst in relation to pregnancy outcome and pre- and post-natal management were evaluated.

Results: In the years 2017-2021, we have examined 5 420 pregnancies and 7 ovarian cysts were detected. Cysts were divided into smaller and larger than 4 cm and simple and complex. 3 cysts were ≥ 40 mm, 4 cysts < 4 cm, 2 cysts were complex, the remaining 5 were simple. Prenatally, conservative management was chosen for all cysts. 4 cysts (57 %) disappeared during pregnancy, 1 after delivery (14 %), in 2 cases postnatal surgical intervention was necessary – 1× cyst puncture and 1× ovariectomy (28 %). Both cysts requiring postnatal intervention were larger than 4 cm, one simple and one complex.

Conclusion: Ovarian cysts are a congenital defect with a good prognosis, majority of them resolves spontaneously during pregnancy or postnatally. The size and type of the cyst are directly related to the likelihood of the cyst disappearing. Our management and results are in agreement with world literature.
