The benefit of magnetic resonance in diagnosis of Ménière's disease

Overview

Objective: Ménière's disease is a complex inner ear disease, that is diagnosed according to diagnostic criteria. Magnetic resonance imaging is a key imaging method, that will allow us to assess the area of the pontocerebel­lar angle, excluding the organic retrocochlear cause of sensorineural hearing loss and dizziness, but may also depict the inner ear area and assess the endolymphatic space. The aim of our pilot study was to evaluate magnetic resonance imaging of endolymphatic hydrops on 3 Tesla (3 T) and 1.5 Tesla (1.5 T) examination in patients with Ménière‘s disease.

Material and Methods: We have started to use magnetic resonance imaging to depict endolymphatic hydrops in patients with dizziness, hearing loss and tinnitus since 2012. Scanning was performed on a 1.5 Tesla device (Toshiba Vantage) after intratympanic administration of contrast medium. We have started to use 3 Tesla device (Siemens Magnetom Skyra) with intravenous application of contrast medium since 2017. The examination gives more detailed and accurate imaging. The big advantage is the sided imaging.

Results: We have included 41 examinations in the study group. The diagnosis of definitive Ménière‘s disease was determined in 18 examinations, in 23 examinations the probable Ménière's disease was diagnosed. Hydrops was detected in higher age categories (68.75% in patients over 60 years). Women had the equal part of proven and unproven hydrops (50% vs 50%). However hydrops was found in 71% of men group. In patiens, who were examined on a 1.5T (20 patients) device, hydrops was detected in 55%. In the group of patients examined on 3 T device (21 patients), the hydrops was more frequent (62%). In the case of definitive diagnosis, hydrops was found in 78%, in probable diagnosis in 44% (p = 0.02).

Conclusion: Diagnosis of Ménière's disease is based on clinical examination and follow-up of the disease. Magnetic resonance imaging can help to confirm the diagnosis. In our study hydrops was detected in 24 examinations (58.5%) and was more frequent in patients with definitive Ménière's disease (78%).
