Overview

The authors emphasize the role of foramen Luschkae in indication of neuro–otologic approached in the therapy of vestibular schwannoma and other disease conditions. The obturation of foramen Luschkae with a medium-size or large tumor may significantly deteriorate circulation of cerebrospinal liquor and just in these cases the lateral neuro–otologic approaches – trans-labyrinth and transotic ones – are apparently least traumatizing.

Precisely in these situations the preoperatively assessed hearing is only rarely socially sufficient. In all other cases, when the circulation of cerebrospinal liquor is not deteriorated, the selection of surgical approach will be specified by the state of hearing on the affected side and character of the disease affecting the pontocerebellar space. The indication of surgical treatment should be selected effectively considering the specific patient.

Key words:
translabyrinthine approach, retrosigmoid approach, foramen Luschkae, vestibular schwannoma.
