What new did the 2025 ESC/EAS update of recommendations for the management of dyslipidemias bring us?

Overview
The ESC/EAS European guidelines for the management of dyslipidemias and prevention of cardiovascular disease have undergone significant developments in recent years. The aim of this review is to compare the 2019, 2021 and 2025 guidelines, focusing on SCORE2 and SCORE2-OP scoring systems, LDL-cholesterol targets, the use of PCSK9 inhibitors, inclisiran in the treatment of dyslipidemias, and the role of lipoprotein(a) in cardiovascular risk stratification. The update of the ESC/EAS guidelines from 2019 to 2025 reflects a shift towards more aggressive, individualized and early treatment of dyslipidemias. Particular emphasis is placed on combination lipid-lowering therapy and on the identification of patients with high genetic risk (familial hyperchoelsterolemia), with the aim of further residual cardiovascular risk reduction.
