Teriflunomide is an oral disease-modifying drug (DMD) used in the therapy of relapsing-remitting multiple sclerosis (RRMS). News Prim. MUDr. Gustáv Ondrejka: Biological treatment of asthma eliminates the need for systemic corticotherapy

Despite advances in the therapy of bronchial asthma, a portion of patients still do not achieve the desired control of the disease. The introduction of biological treatment marked a significant milestone in management, improving the prognosis for patients previously dependent on long-term systemic corticotherapy with all its negative impacts. The recently approved reimbursement for dupilumab expands treatment options for patients with severe asthma, relieving patients of common asthma comorbidities. The current perspective on the treatment of bronchial asthma is provided by the head of the Pulmonary Department at KNTB Zlín, which includes the Center for the Diagnosis and Treatment of Severe Forms of Bronchial Asthma, Prim. MUDr. Gustáv Ondrejka, Ph.D.

News The main advantage of sartans is a very good safety profile. Now it is about increasing adherence in hypertensive patients

The rules for initiating hypertension treatment have been simplified. The trend is to achieve blood pressure control as early as possible, making it unnecessary to wait for a longer period for the effect of non-pharmacological measures. One of the 4 main classes of antihypertensives is AT1 receptor blockers for angiotensin II – sartans, which bring several advantages. These include a low incidence of side effects, a good antihypertensive effect, or the possibility of use in fixed combinations. We will take a closer look at the current position of sartans in the treatment of hypertension with Assoc. Prof. Ondřej Petrák, MD, PhD, from the Center for Research, Diagnosis, and Treatment of Hypertension at the 3rd Internal Clinic - Clinic of Endocrinology and Metabolism at the 1st Faculty of Medicine, Charles University in Prague, and General University Hospital in Prague.

Gliflozins are a relatively new group of antidiabetics with a specific mechanism of action. In addition to glycemic control, they have shown favorable effects on body weight and blood pressure, minimal risk of hypoglycemia, reduced risk of heart failure, and also renal protection and cardiovascular safety. Their position in the therapeutic algorithm for type 2 diabetes is clearly defined in the 2020 recommendations of the Society of General Medicine ČLS JEP.

News Telmisartan – antihypertensive with cardioprotective effects

Sartans are potent inhibitors of the AT1 receptor for angiotensin II widely used in the therapy of arterial hypertension. Given that chronic activation of the renin-angiotensin-aldosterone system (RAAS) plays an important role not only in the pathogenesis of arterial hypertension but also in the pathogenesis of atherosclerosis and the development of other cardiovascular diseases, the question arises whether the administration of sartans is associated with a broader cardioprotective effect. Telmisartan is the first representative of this group of antihypertensives for which a positive cardioprotective effect has been demonstrated.

News Prim. Václava Adámková: People mistakenly believe that there are enough antibiotics and that they will always handle infections

According to the World Health Organization (WHO), by 2050, infections untreatable with antibiotics could become the leading cause of death, surpassing cardiovascular and oncological diseases. Rising antimicrobial resistance was one of the main topics at the VIII Congress of Clinical Microbiology, Infectious Diseases, and Epidemiology (KMINE 2022), held in September in Prague. The topic was also the focus of the expert block ‘ATB stewardship,’ where epidemiologist Petr Smejkal, infectologist Aleš Chrdle, and microbiologist Václava Adámková discussed possible solutions to this global problem. We asked Adámková to summarize insights into the presented issues for us.

News Dravet Syndrome – Get to Know It

Dravet syndrome is a severe myoclonic epilepsy occurring in early childhood. It is classified as a rare progressive epileptic encephalopathy, characterized by generalized or lateralized clonic seizures often prolonged and linked to febrile infections. Other forms of seizures later appear, leading to deterioration in psychomotor development and the onset of mental retardation. Early diagnosis and treatment are desirable.
