TREATMENT OF CALCANEAL FRACTURES – PART I.

Overview

Treatment of calcaneal fractures has received attention since the times Hippocrates (460-385 B. C.). However, despite better diagnostics and treatment strategies, the results are still unsatisfactory. X-ray diagnostics and computer aided tomography have revealed the complexity of the injuries as well as the need for accurate repositioning of the back articulate surface, preservation of the length and width of calcaneal bone together with preservation of Böhler and Gisson angles. Further, views on how to treat calcaneal fractures are rather controversial. Most often discussed are problems in the choice of types of surgery, method and time of repositioning together with choice of osteosynthetic material and the need to fill the calcaneal bone with spongioplastic. Albeit the number of fractures is only between 1-2% of all fractures, it is still the most frequent fracture in the area of the tarsus. Financially, as early as 1946 the cost for calcaneal fracture was 1350 USD. In 1998 at the 69th workshop of AO/ OSIF, Harald Tscherne stated that calcaneal fractures are economically amongst the most demanding fractures in Germany.
