Incidental detection of tumor duplicity on bone scan – SPECT/CT

Overview

70-year old man with prostate cancer was referred to follow-up bone scintigraphy due to the elevated PSA. Whole body scan and SPECT/CT examination revealed slightly increased uptake of the radiotracer in the right frontal area of the skull. The CT image showed bone defect and a dense focus in its proximity which oppressed frontal lobe with perifocal edema. There was no objective neurological impairment, subjectively the patient complained with mild headache and slight vertigo when changing the position of the head. A history of head trauma was negative. The MRI of the head was performed and described extraaxial tumor – meningioma – localized frontally right with diameter of 40 mm, with perifocal edema and incipient left subfalciar herniation. Moreover, another 8 mm small suspected meningioma at falx cerebri was revealed. The patient underwent neurosurgical procedure with the extraction of both tumors. Histological examination confirmed diagnosis of meningiomas. The postoperative course was favorable.

Case report shows the contribution of SPECT/CT in the restaging of patient with prostate cancer, with an incidental detection of tumor duplicity – meningiomas.
