NALIRIFOX in the treatment of metastatic pancreatic cancer: good clinical response at a reduced dose – a case report

Overview
Introduction: Pancreatic adenocarcinoma is one of the most aggressive malignancies and is associated with a very poor prognosis. The novel chemotherapy regimen NALIRIFOX demonstrated improved overall survival and progression-free survival in the NAPOLI-3 trial in the first-line treatment of metastatic disease. Case report: We present a case of a 59-year-old patient with initially metastatic pancreatic adenocarcinoma who was started on palliative chemotherapy with the NALIRIFOX regimen. Despite repeated dose reductions due to treatment--related toxicity, a partial response was achieved, followed by long-term disease stabilisation. The overall duration of treatment response exceeded 18 months. Conclusion: This case report demonstrates the efficacy and clinical applicability of the NALIRIFOX regimen in real-world practice. It also highlights the importance of treatment individualisation and toxicity management, which enable long-term administration of therapy and sustained disease control.
