Overview

Objective: To evaluate patients enrolled in the BIOREP registry of biologic/targeted therapy in the Czech Republic in 2025.

Methods: A retrospective evaluation of patients to date 31 December 2025 enrolled in the BIOREP registry was performed. The aim of our study was to evaluate patients on biological/targeted treatment in the given period in individual categories and to analyze the population of patients with psoriasis, hidradenitis suppurativa and atopic dermatitis. Categorical variables were summarized using counts and percentages. Continuous variables were described using counts, means, standard deviations, medians, minima, and maxima.

Results: As of 31 December 2025, a total of 9,691 patients were registered in BIOREP. Of these, 6,117 patients (63.1%) were monitored for psoriasis, 795 patients (8.2%) for hidradenitis suppurativa, 2,709 patients (28.0%) for atopic dermatitis, and the remaining 70 patients (0.7%) received off-label treatment. Among patients treated for psoriasis, 62.6% were male. The mean age at the end of 2025 was 52.7 years; the mean age at the time of diagnosis was 26.0 years, and the mean age at initiation of first biologic/targeted therapy was 46.4 years. The mean time from diagnosis to initiation of first targeted therapy was 20.4 years. By the end of 2025, patients had been treated with targeted therapy for an average of 5.6 years. Comorbidities were present in 69.8% of patients, with the most common metabolic/endocrine disorders (38.9%) and cardiovascular diseases (36.6%). Among the observed comorbidities, the most frequent were hypertension (33.7%), dyslipidemia (26.3%), and diabetes mellitus (12.4%). Overall, 75.9% of patients were overweight or obese, and 32.8% were smokers. Concomitant psoriatic arthritis was present in 28.3% of patients. Mean PASI at treatment initiation and at the last visit was 18.4 and 1.5, respectively; mean BSA was 28.6% and 2.2%, and mean DLQI was 16.0 and 1.5, respectively. After 12 months of treatment, PASI50 was achieved by 90.2% of patients, PASI75 by 82.2%, PASI90 by 69.5%, and PASI100 by 43.2%. In 2025, targeted therapy was newly initiated in 594 patients. At the end of 2025, a total of 5,241 patients were actively treated, most commonly with risankizumab (Skyrizi, 18.6%), guselkumab (Tremfya, 10.1%), secukinumab (Cosentyx, 9.8%), brodalumab (Kyntheum, 9.7%), ixekizumab (Taltz, 8.5%), and adalimumab biosimilar Yuflyma (7.4%). At the end of the study period, 795 patients with hidradenitis suppurativa were registered, of whom 55.8% were male, with a mean age of 44.8 years. The mean age at diagnosis was 32.2 years and at treatment initiation 41.4 years. The mean time from diagnosis to initiation of first biologic therapy was 9.2 years, and patients were treated for a mean duration of 3.1 years. In 2025, a total of 115 patients initiated their first biologic therapy. At the end of the year, 598 patients were actively treated, with a mean treatment duration per medication of 2.4 years. The most frequently used therapies were adalimumab (Humira, 35.6%), adalimumab biosimilar Yuflyma (26.9%), and secukinumab (Cosentyx, 15.9%). As of 31 December 2025, a total of 2,709 patients with atopic dermatitis were registered in BIOREP, including 49.9% males. The mean age in 2025 was 36.5 years. The mean age at diagnosis was 5.5 years, and the mean age at initiation of first targeted therapy was 34.5 years. The average time from diagnosis to initiation of first targeted therapy was 29.1 years. By the end of 2025, patients had been treated for a mean duration of 2.3 years. Concomitant allergic rhinitis was present in 56.9% of patients, food allergy in 52.6%, and allergic asthma in 37.2%. Ocular involvement was reported in 21.1% of patients, most commonly atopic conjunctivitis (11.2%). Mean EASI at treatment initiation and at the last visit was 29.7 and 3.5, respectively; mean BSA was 55.5% and 6.8%, and mean DLQI was 18.3 and 3.6, respectively. In 2025, targeted therapy was initiated in 612 patients. At the end of the year, 2,409 patients were actively treated, most commonly with dupilumab (Dupixent, 57.8%), lebrikizumab (Ebglyss, 18.3%), and upadacitinib (Rinvoq, 16.5%). The mean treatment duration per agent was 2.1 years.

Conclusion: BIOREP is the first registry of biologic therapy for psoriasis in Central and Eastern Europe. This analysis confirms high disease activity based on objective measures and substantial impairment of quality of life at the initiation of targeted therapy, with marked improvement of evaluated parameters during treatment. It also demonstrates a high prevalence of comorbidities and risk factors, as well as prolonged periods of insufficient treatment prior to the initiation of targeted therapy.
