Hidradenitis Suppurativa: Current Knowledge on Pathogenesis and Treatment

Overview
Hidradenitis suppurativa is a chronic, recurrent, inflammatory systemic disease characterized by painful nodules, abscesses, and sinus tracts in intertriginous areas. The disease significantly impairs quality of life and is associated with numerous comorbidities, particularly metabolic syndrome, cardiovascular diseases, and inflammatory bowel diseases. The pathogenesis of hidradenitis suppurativa is multifactorial and involves dysfunction of the follicular unit, activation of innate and adaptive immunity, and chronic inflammation leading to tissue destruction. Diagnosis is clinical, and disease severity is primarily assessed using the Hurley classification and the International Hidradenitis Suppurativa Severity Score System (IHS4). Current treatment includes topical therapy, systemic antibiotic therapy, biologic therapy targeting TNF-α and IL-17, and surgical removal of irreversible lesions. The goals of treatment are suppression of inflammation, prevention of tissue destruction, pain control, and improvement of patients’ quality of life. This review summarizes current knowledge on the epidemiology, pathogenesis, and comorbidities of hidradenitis suppurativa and proposes a practical therapeutic algorithm for outpatient dermatological practice.
