Overview

The systemic inflammatory response enables the host to survive otherwise lethal conditions such as serious infectious diseases, multiple injuries, or complicated operations, an example of which are cardiac surgical operations with the use of extracorporeal circulation. A successful outcome of the systemic inflammatory response is provided by synchronized activation of both its pro-inflammatory and its anti-inflammatory components. Exaggerated activation of its pro-inflammatory part presents high risk for the host of being exposed to toxic effects of the pro-inflammatory cytokines which may lead to the development of the multiple organ failure syndom. An integral part of the inflammatory response is synthesis of anti-inflammatory mediators which in a natural way attenuate the activity ofinflammatory and immune reactions. Interleukin-10 is considered the most powerfizl anti-inflammatory cytokine. The therapeutic potenciál of interleukin-10 and its analogs is in its ability to protect the host from unwanted activity of inflammatory reactions including protection of ischaemic organs from reperfizsion injury. Exaggerated activation of the anti-inflammatory reactions with inappropriate synthesis of IL-10, on the other band, presents a menace to the host in the form of uncontrolled dissemination of pathogenic mikroorganisms with increased mortality from infectious diseases.

Key words:
systemic inflammatory reaction - proinflammatory cytokines - antiinflammatory cytokines - cardiac surgical operations - ischaemia and reperfusion.
