Výskyt děložních sarkomů v západních Čechách, Gynekologicko-porodnická klinika LF UK a FN Plzeň

Souhrn

Cíl studie, název a sídlo pracoviště: Cílem této studie na Gynekologicko-porodnické klinice ve FN Plzeň bylo provedení retrospektivní analýzy dat našich pacientek s děložními sarkomy za účelem bližší charakteristiky této skupiny. Studie pokrývala období od 1/2005 do 1/2015, včetně údajů follow-up těchto pacientek do 1/2017.

Metodika:
V rámci analýzy byla retrospektivně zhodnocena tato data a možné rizikové faktory: věk pacientky, histologický typ a velikost tumoru, FIGO klasifikace, body mass index, předchozí ozařování či užívání tamoxifenu atd. Od 1/2005 do 1/2015 bylo diagnostikováno 20 děložních sarkomů. Studie zohledňuje data z follow-up těchto pacientek do 1/2017.

Výsledky:
V naší skupině bylo diagnostikováno 12 leiomyosarkomů, sedm endometriálních stromálních sarkomů a jeden případ nediferencovaného děložního sarkomu. Všechny pacientky, bez ohledu na histologický typ sarkomu, dosáhly dvouletého přežití v 50 %. Medián doby do návratu onemocnění byl 18 měsíců, medián follow-up 38 měsíců.

Postmenopauzální krvácení bylo přítomno v 55 % případů. U 25 % pacientek byla diagnóza sarkomu známa již předoperačně z histologického výsledku kyretáže, 10 % případů mělo ultrazvukový nález se suspekcí pro sarkom. Ve 48,2 % byl endometriální stromální sarkom asociován s multiparitou.

Závěr:
Děložní sarkomy jsou vzácné agresivní tumory, což platí i pro populaci žen v západních Čechách s velmi vysokou incidencí děložních malignit. Ve dvou případech došlo k morcelaci dělohy skalpelem při vaginální části LAVH (bez znalosti diagnózy děložního sarkomu). Tento fakt se však neprojevil negativně v následujícím období sledování.
